<problem_statement>Millions of patients still receive their lab results, imaging reports, and prescriptions as unstructured PDFs, photos, or scanned documents.
Clinics and hospitals waste hours manually reading, interpreting, and entering this information into systems. Often leading to delays in care, transcription errors, and loss of follow-up.
Patients also struggle to organize their own medical documents, especially when they come from different providers or labs.</problem_statement>

<h3>Which ServiceNow products or capabilities would be used?</h3>
<p></p><p>Patients can securely upload their medical documents directly through a <strong>Portal</strong> or <strong>Mobile App</strong>. Using <strong>Document Intelligence</strong>, the system automatically scans, classifies, and extracts key information (e.g., test type, values, diagnosis notes, physician name, date). The extracted data is then stored in a structured format within the <strong>Healthcare and Life Sciences Service Management (HLS)</strong> module.</p>
<p>Healthcare professionals can access this data through a unified dashboard that highlights trends, alerts for abnormal results, and integrates seamlessly with <strong>Patient Case Management</strong>, <strong>Care Coordination</strong>, and <strong>Knowledge Management</strong> for clinical reference.</p>
<p>The platform would also leverage <strong>Now Assist</strong> for summarization and insights, helping clinicians quickly understand patient history, and <strong>Flow Designer</strong> for automated notifications, such as reminders for follow-up visits or alerts about missing information.</p>
