<problem_statement>In community home/long-term care (LTC) and community health settings, early identification of residents who need palliative care is often delayed. This causes unnecessary suffering for residents, increased distress for families, and inefficient coordination among nurses, physicians, and personal support workers.
The current process depends on manual reviews and fragmented documentation, which makes it difficult to track residents’ changing conditions in real time and trigger timely palliative consultations.</problem_statement>
