Subject: [PATCH] Describe the watch bolus that opens on the amount and starts
 at the recommendation

Matches loopandlearn/LoopFollow#792: the bolus screen opens on the amount,
starts at a recommendation under 5 minutes old and follows newer loop
cycles until the amount is changed by hand. Quick-Pick boluses moved to
Presets, and the confirmation checks the recommendation again before
sending.
