修复 #1432：
TokenUsageExtractor只通过getattr读取 usage，provider 以 dict（如usage_metadata={...}）返回时 token 计数全部为 None。本次让_extract_from_usage_object同时支持 Mapping，用.get()读取，并补充对应单元测试。验证：本地已验证 dict 与 attribute 两种形态；新增用例交由 CI 执行。
