fix(pr-review): re-read the whole diff after a fix instead of holding confidence at 8 forever - #2019
A pull request whose earlier review found a defect was capped at 8 on every later round, even after the fix landed and the latest pass was clean ("Held at 8 because earlier reviews of this pull request found 1 defect"). Authors read that as being penalized for fixing the bug.
The cap had a real reason: follow-up rounds only read the files changed since the last review, so nothing ever re-checked the rest of the PR around a fix. The cap stood in for a check we never ran.
Change
Run that check, then drop the cap once it passes.
The round after a fix reads the whole diff.followUpFor loads the PR's completed reviews. If a defect was fixed after the last whole-diff pass, the kickoff says "review the whole diff" instead of listing changed files. The coverage gate already treats a kickoff with no scope as "every file is due".
Whole-diff passes are recorded. The runner stamps wholeDiff: true on the report when every file was due and read. A close-out that files a stopped pass's findings, or a pass that left files unread, is not stamped.
The cap counts only unchecked fixes.confidencePrReview's history cap now takes defects fixed since the last whole-diff pass (uncheckedFixes), not every defect the PR ever had. A clean whole-diff pass scores 10, and later incremental pushes stay there. The comment now reads "Held at 8 because a fix has not been re-checked against the whole diff yet."
Reviews stored before this change have no flag, so the first push after deploy on a PR with a past fix gets one whole-diff pass.
Cost
One extra full read per fixed defect, about 20 to 50 cents (up to about $1 on large PRs). Worst case across the org is about 12% on top of current review spend, probably less in practice. Every other round stays incremental.

Tests
uncheckedFixes: counts fixes since the last whole-diff pass, counts none once a whole-diff pass has run (including one that fixed something itself), counts every fix on reviews stored before the flag, and ignores open, dismissed and non-defect findings.
Kickoff and coverage: after a recheck, the kickoff has no scope and every file is due.
Runner: wholeDiff is stamped only when every file was due and read, and never on a close-out.
Service end to end: a fix in an incremental round is held at 8. The next push asks for the whole diff, and a clean pass lifts the cap. The push after that is incremental again and stays uncapped.
… confidence at 8 forever
A pull request whose earlier review found a defect stayed capped at 8 on every
later round, even once the defect was fixed and the latest pass was clean. The
cap existed because follow-up rounds read only what changed since, so nothing
ever re-checked the rest of the pull request around the fix.
Now the round after a fix is due the whole diff. A pass that reads every file
is stamped wholeDiff on its report, and the cap counts only defects fixed since
the last such pass, so a clean whole-diff pass reads 10 and later incremental
pushes stay there.

Walkthrough
The review flow now records whole-diff coverage, requests a whole-diff recheck when resolved findings remain unchecked, and tracks those fixes in confidence calculations. A whole-diff review resets the unchecked-fix count.
ReviewCoverage now reports whether the review has a narrower scope. Submitted reports set wholeDiff only when coverage is whole-diff, the submission is not partial, the verdict is not not_applicable, and no files remain unread. Tests cover the coverage signal and report conditions.
Track fixes and request rechecks packages/backend/src/services/pr-review/findings.ts, packages/backend/src/services/pr-review/findings.test.ts, packages/backend/src/services/pr-review/PrReviewService.ts
Tracked findings include their resolution head SHA. Review history identifies resolved defect findings since the latest whole-diff pass. Follow-up prompts request a whole-diff recheck when fixes remain unchecked.
Apply unchecked-fix confidence cap packages/backend/src/services/pr-review/PrReviewService.ts, packages/backend/src/services/pr-review/review-signal-corpus.ts, packages/domain/src/http/pr-review.ts, packages/domain/src/http/pr-review.test.ts, packages/backend/src/services/pr-review/PrReviewService.test.ts
Confidence calculations use the unchecked-fix count and retain the history cap until a whole-diff pass. A whole-diff report resets the count. Tests cover successive incremental and whole-diff reviews.
Follow-up reviews may lift the confidence cap after a fix without having read the whole diff. This happens when large patches or very large file listings are truncated, or when a reviewer requests a repeat of the same commit. The effect is overconfident review scores in those cases, not an outage. Closing these gaps is advisable before merge.

Full details: Title check
Explanation
The title names the fix in the pr-review follow-up logic: after a defect fix, the next review reads the whole diff, and the confidence cap of 8 is lifted only after a clean whole-diff pass. The changeset matches this: it adds wholeDiff tracking, uncheckedFixes counting, a recheck kickoff, and a confidence cap change. The title states the main behavior change in one sentence, so it passes. It is somewhat long at 93 characters, but it stays specific and does not use vague terms.

…ad the flag in SQL
A pass whose file listing never arrived owes no files, so an empty unread list
proved nothing; coverage now requires a listing before it reports wholeDiff.
The review history selects the flag from the stored report in SQL instead of
decoding every report of the pull request.
F2 (loadReviewHistory loading every report): fixed in 1373f65. The history now selects only (report_json->>'wholeDiff')::boolean in SQL, the same way PrReviewAnalyticsService reads report fields, so no report bodies are loaded or decoded.
🟢 Confidence 8/10 · likely safe to merge. Held at 8 because earlier reviews of this pull request found 2 defects. no findings · tests covered · risk medium · 1/1 new units observable · 2 defects fixed in earlier rounds
A kickoff after a fix now asks for the whole diff, the runner stamps wholeDiff only when every due file was listed and read, and the confidence cap counts only fixes since the last whole-diff pass. The two earlier findings are fixed and the new paths are tested.
uncheckedFixes counts only fixes made since the last whole-diff pass
The pass after such a fix gets a kickoff asking for the whole diff
wholeDiff is stamped only when every due file was listed and read
The history cap drops once a clean whole-diff pass has run

Preserve unchecked fixes when re-reviewing the same head.
If head B resolves a finding and a user requests another review of B, reclaimFailed changes B's row to queued before this history query. uncheckedFixes then cannot match the finding's resolvedSha to B, so the kickoff requests an incremental review. The submission calculation also misses the fix and can lift the cap without a whole-diff pass. Preserve the previous completed review state when reclaiming the row, or otherwise include its unchecked fixes in both calculations. (github.com)

Update the re-review history handling around uncheckedFixes so reclaiming a
failed row for the same head does not discard its prior completed review state;
ensure the preserved unchecked fixes are included in both the kickoff decision
and submission calculation.

Ignore truncated file listings when setting wholeDiff.
pr_changed_files lists at most 500 files, and the listing is cut before reviewable files beyond that point are shown. makeReviewCoverage.wholeDiff() returns true whenever any listing arrived. On a pull request with more than 500 changed files, a review can read every listed reviewable file and still be stamped wholeDiff: true. The confidence cap is then lifted without the unlisted files being read.
Make wholeDiff() return false when the listing reports fewer files than the total. Track that truncation in observe for pr_changed_files, and keep the cap in place until a listing covers every changed file. Confirm the exact truncation wording in the renderer before matching it.

Major · Functional Correctness · Heavy lift
Do not count incomplete file diffs as read.
pr_file_diff truncates large patches, but coverage marks a file read from its heading alone. Coverage also excludes files marked no patch, including oversized source files. Either case can stamp wholeDiff: true and lift the cap while code remains unread. Track incomplete patches separately and require evidence that their remaining content was reviewed.
