Add versioned knowledge-pack/ with seed allowlist/denylist, exclusion data, notes frontmatter, and meta; pure fail-closed policy evaluators in sidecar with 22 acceptance tests for URL deny, mixed citation refuse, and same-name Apply refusal. Co-authored-by: Cursor <cursoragent@cursor.com>
