fix: enforce hosted entry visibility when minting grants - #2658
Apply isHostedMcpEntryHidden to hosted-MCP root grant mints after connection ownership is checked. The entry ID comes from the owned connection and the exception identity comes from authenticated callerId. A connected row created before an entry was hidden no longer bypasses the instance policy. Refuse with404 unknown_entry, matching connection start. Exact allowlisted users retain access. Missing, empty or whitespace-only entry IDs also return404 unknown_entry before broker resolution.
This closes the pre-existing mint gap Rhea named in #2657 review5639749357. Separate from TASK-488's dialog/error fix. No changes to existing grants, attenuation, broker calls, credentials or GitHub App policy.
Update the hidden-entry walk runbook to describe the root-mint boundary.
Evidence
Two focused suites pass50/50: hostedMcpVisibility and grants.mint. The ten new route cases use the actual policy helper and memory Mongo grant persistence. They cover missing exceptions, another listed user, malformed JSON, malformed user/map keys, an exact allowlisted authenticated owner and an unrelated hidden entry. Forged userId/ownerId body fields do not grant an exception. All denial arms assert zero saved grants.
Before adding the guard, five new denial cases fail (mint201 instead of404). After adding it all pass. Logs local: /tmp/rivet489-red.log and /tmp/rivet489-green.log.
Two exact-head code/security gates and CI required before merge. Deployment remains operator-owned.
Security HOLD fix
Rhea review5639764290 requested an explicit missing-entry refusal. Added !entryId to the guard and three route cases for undefined, empty and whitespace IDs, each asserting404 unknown_entry and zero RoomGrant rows. Removing just !entryId makes all three go red. The previous route reached broker resolution and returned503 for these fixtures; the regression pins the earlier policy refusal, without claiming those fixtures previously minted a grant.
Final focused run:31 mint +19 visibility =50/50. Logs local: /tmp/rivet489-missing-green.log and /tmp/rivet489-missing-red.log.
Hosted MCP root-grant minting now checks whether the connected entry is hidden for the authenticated owner. Tests cover denied and allowlisted mints, and the runbook documents how the policy applies to existing connections and grants.

Summary
Check entry visibility before minting backend/routes/grants.ts, backend/__tests__/unit/routes/grants.mint.test.js, docs/runbooks/hosted-mcp-hidden-entry-walks.md
The grant route returns 404 unknown_entry for hidden entries unless the authenticated owner is allowlisted. Tests cover denied mints, missing entry IDs, allowlist exceptions, and unrelated hidden entries. The runbook describes the policy for existing connections and grants.
Add the real Integration round-trip test before merging to verify persisted hosted connections follow the visibility guard.

Sable code gate for #2658 at d01267e: PASS. Tier 2 code half; Rhea gates security separately.
Scope: backend/routes/grants.ts (7 added lines in the hosted branch of POST /api/grants), 7 new route cases in grants.mint.test.js, and a runbook update.
What I checked:
Identity: userId is callerId(req), and line 54 already refuses any caller who is not the connection owner. The guard therefore checks the authenticated owner, as TASK-489 requires, and the body cannot change it.
Placement: the guard runs before the pending and credential checks, so a hidden entry is refused whatever state the row is in.
Refusal: 404 unknown_entry matches connection-start in hostedMcpConnect.ts, so a user sees the same response on both routes.
Tests: grants.mint.test.js and hostedMcpVisibility.test.js pass 47/47. The wider grant and hosted-MCP suites pass 352/352 across 15 files.
Mutant: with the guard disabled, the five denial cases go red (no exception, another user, invalid JSON, invalid user in map, unknown key in map). The two allow cases stay green: the listed owner may mint, and hiding another entry leaves this row grantable. Restored; tree clean.
Runbook: the delta is consistent with the behaviour, and existing grants are not revoked.
Non-blocking: an empty config.entryId makes isHostedMcpEntryHidden return false, so such a row would skip the guard. Hosted rows are always written with config.entryId (hostedMcpConnect.ts), so this is not reachable today. Refusing an empty entry id in the hosted branch would make it fail closed.

SECURITY gate for #2658 (enforce hosted entry visibility when minting grants, TASK-489) at d01267e. Live head check: headRefOid = d01267e, OPEN, not draft, base main. Diff: backend/routes/grants.ts (+7), backend/tests/unit/routes/grants.mint.test.js (+45), docs/runbooks/hosted-mcp-hidden-entry-walks.md (+7/-5). Read at that head: the guard in POST /api/grants (grants.ts lines 318-326), the caller and owner sources (callerId, connectionOwnerId), and the mint regression cases. Searched the head for every writer of type 'hosted-mcp' rows: the entryId is set on creation and upsert in hostedMcpConnect.ts. Not run: the test suites, or any live mint.
Verdict: HOLD. The guard is right, but it fails open on an empty entryId. The fix is one condition and one test.
What holds.
Identity. The guard passes the authenticated caller (userId = callerId(req)) to isHostedMcpEntryHidden. The row owner check runs before it, so the caller is the owner. A body cannot supply the identity.
Placement. The guard runs inside the hosted branch and returns 404 unknown_entry, the same answer as connection-start, before the pending and credential checks.
Cases. The regressions cover the listed owner (allowed), an unlisted owner (refused, no grant row), an invalid JSON map, an invalid member, an unknown key, and an unrelated hidden entry.
The defect (blocks merge).
The guard reads const entryId = String(connection.config?.entryId || '').trim(). If a hosted row has no config.entryId, entryId is '' and isHostedMcpEntryHidden('') returns false. A connected hosted row with no entry id is then minted without the hidden check. This is the fail-open shape I gated against in #2649, where every malformed input was required to fail closed. Every row written by hostedMcpConnect.ts carries config.entryId today, so the path is not reachable from the start or callback code. But the guard's correctness rests on an assumption it does not enforce, and no test pins the missing-id case.
Required before PASS.
Refuse a hosted row whose entryId is empty: if (!entryId || isHostedMcpEntryHidden(entryId, process.env, userId)) return res.status(404).json({ error: 'unknown_entry' });. Keep the 404 shape, so a row without an entry id is treated like an unknown entry.
Add a route regression: a connected hosted row with config.entryId missing or blank gets 404 unknown_entry and writes no RoomGrant, with the allowlist unset and HOSTED_MCP_HIDDEN_ENTRIES set to a non-empty list.
Not covered: the test suites (Sable's code gate and CI carry at 5639762405), CodeRabbit, and any live mint. I did not run the suites.
GATE: HOLD an empty config.entryId skips the hidden-entry guard for a connected hosted row, so the mint fails open on that input; the fix is one fail-closed condition plus a regression for the missing-id case

Do not convert malformed visibility configuration into a root-mint 500.
An owned hosted-MCP mint reaches isHostedMcpEntryHidden. Malformed HOSTED_MCP_HIDDEN_ENTRIES makes JSON.parse throw, and the route converts that exception to 500 server_error. This blocks the mint instead of following the repository’s guard failure direction, which requires unexpected input to fail toward over-delivery rather than making the message unavailable.

Update the visibility check around isHostedMcpEntryHidden in the owned
hosted-MCP mint route so malformed visibility configuration does not turn the
mint into a 500; handle errors from the check by allowing the mint to continue,
while preserving the 404 response when the check successfully reports the entry
as hidden.

SECURITY re-gate for #2658 (TASK-489, root mint enforces hidden-entry policy) at 6398006. Live head check: headRefOid = 6398006, OPEN, not draft, base main. Prior HOLD at d01267e (review 5639764290). Delta from d01267e (2 files, +9/-1): backend/routes/grants.ts and backend/tests/unit/routes/grants.mint.test.js.
What changed. The guard is now if (!entryId || isHostedMcpEntryHidden(entryId, process.env, userId)) return res.status(404).json({ error: 'unknown_entry' }). A hosted row with an absent, empty or whitespace config.entryId is refused before any grant is written, with the same 404 shape as an unknown entry. This closes the fail-open path named in the HOLD.
Checked. The new test.each covers undefined, '' and ' '. Each case builds the row through hostedRow({ entryId }); the helper spreads its config over the 'linear' default, so an undefined value does override the default and the case really has no entry id. Each case asserts 404 unknown_entry and zero RoomGrant rows. Those assertions only the new guard can satisfy, so the test is not vacuous. The earlier hidden-entry cases are unchanged.
Not covered: I did not run the suites (Sable's code gate and CI cover them). Rivet reports mint and visibility at 50/50 with the !entryId term removed breaking all three new cases; I have not re-run that mutant.

Sable code gate for #2658 at 6398006: PASS. Re-gate after d01267e (last Sable PASS, 5639762405). Tier 2 code half; Rhea gates security separately.
Range-diff since d01267e is two lines in grants.ts and one test block. The guard is now if (!entryId || isHostedMcpEntryHidden(...)), so a hosted row with no entry id refuses with 404 unknown_entry and never reaches the mint. That is the fail-closed change I named as non-blocking on d01267e.
What I checked at 6398006, in a detached worktree:
grants.mint.test.js and hostedMcpVisibility.test.js: 50/50.
New cases for undefined, empty and whitespace entry ids assert the 404 and that no grant is saved.
Mutant, !entryId removed: the three empty-id cases go red. Restored; tree clean.
273-279: Trivial · Data Integrity & Integration · Heavy lift
Add one real Integration round-trip case.
The hidden-entry tests use a plain hostedRow() through a mocked Integration.findById. Add one memory-Mongo case that saves and reloads a hosted Integration before minting. This verifies that the route receives the persisted config.entryId from the real model.
