fix(write-to-file): run diff cleanup when handleError rejects (split 5/6 of #1066) - #1932
Part of the upstream PR 1066 split. Own issue: 1937. Content source of record: 72143527fd33306e5541116093c2cbf803cce9e0..pr-1066-audit).
Why this unit exists: execute() cleanup invariant: the finally around handleError, the writeApproved flag and the consecutive-mistake-counter order. Soft budget overshoot: 439 a+d (single provider group, single gate scope).
src/core/tools/WriteToFileTool.ts: OK (content subset of source)
src/core/tools/__tests__/writeToFileTool.spec.ts: OK (content subset of source)
Budget rationale (soft overshoot): see the deviations list
Design contract
issue: the split plan
Chain position
Merge order is fixed: U12 (1927) -> U4 -> U5 -> U3 -> U6 -> U7 -> FINAL (1928). This PR is opened against main because the split branches live on the fork; the diff GitHub shows is therefore cumulative through this unit. The unit's own content is the delta from the previous unit head (U3), listed under Fidelity above. The sole merge target of the series is the FINAL integration PR (1928); merging the chain in order keeps every bot-visible diff clean.
ESLint: clean on every touched file; suppression counts unchanged
No .changeset file, no CHANGELOG edit.
Recreate policy
If the bot stalls on a pre-merge check and the existing head cannot obtain bot review/approval (empty-commit re-trigger attempted and failed), the unit is recreated from the tagged content source of record — never from a per-PR head. At most 1 PR per issue.

Round update — Lifecycle Resource Cleanup
Shared root cause behind the Lifecycle Resource Cleanup row (all five units of 1066).handlePartial() registers this task's partial-stream entry — and its TaskAborted listener — before it checks the prevent-focus-disruption experiment. With the experiment enabled the delta returns without ever showing a preview and never reaches execute()'s teardown, so the entry and the listener stay attached for the rest of the task's life, and a streamFailed mark armed by an earlier failed delta keeps suppressing this task's later diff previews. The sibling units carry the same release in their own PRs, each verified red-first with a negative control.
This unit (U6):execute() is already covered by the finally { this.resetTaskPartialState(task) } block, so only the suppressed-preview return in handlePartial() lacked a release; it now has one.
Red first: the new test failed with expected 1 to be +0. Green: 57 passed / 5 skipped. Negative control: removing the release turns exactly that one test red; restored green.
Main refresh. Merged org main 036245c5e (U1 1927). U1's content no longer appears in this diff: 10 files +2048/−45 → 7 files +1593/−40, 0 behind main. Conflicts were confined to src/core/task/__tests__/Task.spec.ts (and Task.ts on U7) — the region U1 rewrote; resolved by taking main's version of the shared save-stage tests (try/finally plus the fixed-task-id ui_messages.json cleanup from cf9206a42) rather than re-implementing U1.
Verification after the merge: Task.spec 172 passed, writeToFileTool.spec 57/5, eslint 0/0.
Scope declared in the split plan
CodeRabbit's Out of Scope Changes row asks to either narrow this PR to the execute() cleanup scope or update the linked split. The plan was updated first, per this chain's planned-and-issued rule - see, comment 6076465965 (2026-10-09), which declares U6's scope at head c6b77fbce. The files outside the execute() cleanup scope are therefore planned, not drift:
src/core/tools/BaseTool.ts - partial-stream state ownership: the per-task taskPartialStreamState map and its release pair live in BaseTool, so the prevent-focus-disruption release cannot be expressed without touching it.
src/core/webview/ClineProvider.ts + src/__tests__/removeClineFromStack-delegation.spec.ts - teardown wiring: the shared finalization and the diff-view reset the cleanup path calls are owned there.
partial-stream lifecycle in handlePartial() - it registers the per-task entry and the TaskAborted listener; releasing that registration is the defect this unit fixes.
Chain merge order: U1 1927 (merged) -> U2 1928 -> U3 1931 -> U4 1929 -> U5 1930 -> U6 1932. The same root cause is fixed once and ported; every port commit cites the source commit.
Rows from the 2026-10-09 review
Two rows in that review need files outside the execute() cleanup set. The scope was extended in the split plan before the fix was pushed: (retired fork tracking item 41).
Persistence Integrity - DiffViewProvider.revertChanges() must roll the created file and directories back even when open() failed before activeDiffEditor was assigned. Same root cause as 1930; ported verbatim from 7b783452c / 6cae369d9.
Lifecycle - handlePartial() re-checks that this task's stream entry is still live after each await (provider state, filesystem probe, partial ask, open()).
Regression Evidence - the missing-content execute() test, same shape as the three on 1929.
Fix commit: 443993e57. Negative controls are one-to-one (each re-check and each release site kills exactly its own test).
Split-unit issue reference
The linked issue 1937 is the approved issue for this change. It is the unit issue filed under the declared split of a larger change: the split plan, the unit boundaries, the approval of the split, and this unit's acceptance criteria are recorded on the tracking issue in this repository: 1989. That tracking issue is also the home for this unit's review dispositions and follow-up registrations. "No approved upstream issue" would only mean the original single-PR change, which was split before any of these PRs opened.
Stryker directive sweep: removed all three // Stryker disable next-line comments in WriteToFileTool.ts per maintainer directive (mutation-diff is advisory; suppression comments are no longer accepted). Equivalence rationale preserved here:
ConditionalExpression (path-stabilization check): the !== undefined clause is redundant - when lastSeenPartialPath is undefined the second clause only matches an undefined partialPath, which the !!partialPath in the return value rejects either way.
AssignmentExpression (commit-point rollback stand-down): the approval flag already stands the rollback down, so removing the assignment changes no observable behaviour today; it stays because this branch lands the write without a diff view and the commit point is the fact that says so.
LogicalOperator (rollback guard): writeCommitted is the second, independent stand-down - every path that sets it has already set writeApproved, so no test can tell && from || here; it stays because it names the durable fact the provider signals at the save, not the approval fact, and it is the guard that survives if the approval flag moves. CR round (Data-Integrity MAJOR, refused-restore save): restorePreStreamBuffer() ignored the boolean from vscode.workspace.applyEdit; a refused edit left the streamed content in the buffer and saveBufferClean() persisted it - unapproved output on disk, worse when the close also failed because discardFileTab() then throws before removeCreatedFile(). The restore now reports whether it applied, the save is gated on it (mirroring discardUnapprovedStream()), the close-failure message distinguishes an un-restored buffer, and the misleading comments are corrected. New focused test mocks applyEdit resolving false and asserts no save plus the filesystem rollback still running (negative control: exactly one red with the fix stashed). Verification: DiffViewProvider suite 99/99, ESLint --max-warnings=0, tsc --noEmit and Prettier clean.
CR round 3 (Data-Integrity MAJOR, close over unconfirmed buffer): the refused-restore guard fixed the save but not the close. Both failure modes now stop the rollback and report it: a refused restore throws before tabGroups.close() (closing the dirty tab would trade the rollback report for a save-confirmation prompt over unapproved content), and saveBufferClean() returns document.save()'s confirmation - an unconfirmed clean state stops the close the same way. The close-failure message no longer needs the restored/un-restored branch. Tests: refused restore -> no close + rollback-failure report; save() resolving false -> no close + report (negative controls: exactly one red each with the fix stashed; the existing refused-close test's save double now resolves true, matching the confirmed-save precondition). Verification: DiffViewProvider suite 100/100, ESLint --max-warnings=0, tsc --noEmit and Prettier clean.
Lint follow-up: upstream main re-enabled @typescript-eslint/no-unused-vars for src, which exposed dead code in this branch on the merge ref. Cleared: the unused stateFor helper in writeToFileTool-partial-state-cleanup.spec.ts, and the default onParameterParseFailure implementation's three ignored parameters in BaseTool.ts now carry the rule's _ prefix (JSDoc updated to match). Focused suites re-run green; ESLint --max-warnings=0, tsc --noEmit and Prettier clean.
Updated this round (Persistence Integrity check): the walkthrough at the previous head reported that reset() awaited the diff-view close before clearing the session with no finally, so a rejected close left relPath, editType, isEditing and the placeholder ownership behind; the next execution then skipped open(), reused the stale create session, and a later denial's rollback unlinked the path derived from the stale relPath - the file the previous write had approved. All three claims verified real at that head (not stale): the missing finally, the write-path caller that swallows the reset error, and the rollback deleting the relPath-derived path. Fixed: reset() now clears the whole session in a finally while still propagating the close failure, and the rollback deletes only the placeholder this edit owns, snapshotted and cleared before any await. A regression test covers the approved-write, failed-reset, denied-write sequence (negative control: exactly one red with the source fix stashed). DiffViewProvider suite 101 passed; ESLint max-warnings=0 clean with the file's suppression count unchanged; typecheck and Prettier clean.
Updated this round (main refresh): merged upstream main (8018cba40) into the branch; the merge itself was clean. The merge-ref compile job then flagged six test doubles this branch added to the write-tool spec: main made toolCallId a required member of ToolCallbacks, and those literals predate the change. Added the missing toolCallId to them, matching the value main's own doubles in the same file use. Verification at this merge: typecheck clean; write-tool spec 68 passed / 5 skipped; DiffViewProvider spec 101 passed; ESLint max-warnings=0 clean; Prettier clean.
Updated this round (re-review follow-ups): three review threads addressed. (1) The modify branch of the rollback no longer returns early when the editor never opened - only the document work is skipped, so the shared preview-tab restore and session reset always run and a failed open cannot leave stale session state behind; the no-editor spec now asserts the session is released (negative control: exactly one red with the fix stashed). (2) The unapproved-stream discard now reuses the shared ENOENT-tolerant file/dir removal helpers instead of inline copies; mutating the shared helper to drop the tolerance turns exactly one existing discard test red, proving the reuse is covered. (3) The delegation spec now asserts the release detaches the registered abort listener with the same function reference, not just the map entry (negative control: deleting the detach call turns exactly that test red). Verification: DiffViewProvider suite 101 passed, delegation suite 24 passed, typecheck clean, ESLint max-warnings=0 clean, Prettier clean.
Updated this round (Persistence Integrity check, denial path): the new-file rollback reached the delete with the streamed content still in the diff tab's modified side - the tab is a TabInputTextDiff that the file-tab discard does not match, and the diff-view close skips dirty tabs, so the placeholder was unlinked underneath an open dirty buffer one save away from recreating the deleted file. The rollback now restores the modified side to its pre-stream content and confirms a clean save before any tab or file goes, then closes the diff tab and only then unlinks; a refused restore or an unconfirmed save stops the rollback before the close and the delete and propagates. New regression test drives the denial through a dirty TabInputTextDiff and asserts the order restore -> save -> close -> unlink with only the empty pre-stream content ever written (negative control: exactly one red with the source fix stashed); the pre-existing denial test was updated to the restore-then-save contract with its own negative control. Suite 102 passed; typecheck, ESLint max-warnings=0 and Prettier clean.
Updated this round (Persistence Integrity check, commit path; Regression Evidence warning): saveChanges() awaited the document save without checking its boolean result, so a save resolving false still crossed the commit point - onCommit ran and placeholder ownership was released although nothing had persisted. The commit point now requires a confirmed save; an unconfirmed one throws before onCommit, leaving the uncommitted cleanup state (placeholder still owned) for the execute error path, which already handles a pre-commit saveChanges rejection without reporting success. Both coverage gaps named by the warning are closed: a focused open() test rejects the placeholder write and asserts ownership stays unset while the rollback cleans created directories without unlinking the unowned path, and a clean-document saveChanges() test asserts the commit callback runs and ownership is released without a save. Each new test carries a negative control (exactly one red under the corresponding mutation). Suite 105 passed; typecheck, ESLint max-warnings=0 and Prettier clean.
Updated this round (Persistence Integrity check, execute path): the execute path conflated approval with a confirmed commit - writeApproved was set before saveChanges(), so a save-confirmation failure before onCommit skipped the discard and reset() cleared the placeholder ownership anyway, stranding the empty placeholder on disk unowned where a later attempt would mistake it for the pre-existing target. The execute path now distinguishes the two: when saveChanges() fails before onCommit, the owned empty placeholder is removed before reset() forgets the ownership (ENOENT-tolerant release), while the dirty editor keeps the approved content for a manual save or retry and the created directories stay in place for that save. New execute-level regression test (save resolving false: no success result, placeholder removed before reset, buffer not discarded) plus provider unit tests for the release, each with a negative control. Suites 176 passed; typecheck, ESLint max-warnings=0 and Prettier clean.
Updated this round (Persistence Integrity check, placeholder-removal failure): a placeholder removal that rejects (EPERM/EACCES) was logged only, and the cleanup then reset the session anyway - forgetting the ownership and stranding the empty placeholder unowned with no user-visible hazard and no retry path. The execute path now treats a failed removal as a rollback hazard: reset() is skipped so the provider keeps owning the placeholder and the diff session stays open for a retry, and the hazard is reported in the chat like a failed rollback. New execute-level regression test (pre-commit save failure followed by a rejecting removal: failure surfaced, no success result, reset cannot strand the placeholder) plus a unit test pinning that a rejecting unlink preserves ownership, each with a negative control. Suites 178 passed; typecheck, ESLint max-warnings=0 and Prettier clean.
Updated this round (Persistence Integrity check, rollback ownership): revertChanges() and discardUnapprovedStream() released placeholder ownership up front, before the awaited restore, tab cleanup and unlink - a denied new-file edit whose rollback then failed left the empty placeholder on disk unowned with no hazard and no retry path. Ownership is now released only when the unlink succeeds; execute() skips reset() while the provider still owns a placeholder (new ownsPlaceholder signal) and reports the hazard in the chat, the same discipline as the pre-commit save-failure path. The two coverage gaps the review flagged are closed: handlePartial releases the stream state when the filesystem probe rejects, and a rejecting hazard report is logged without aborting the teardown. Six new tests, each with a negative control (exactly one red). Suites 184 passed; typecheck, ESLint max-warnings=0 and Prettier clean.
Updated this round (Persistence Integrity check, unconfirmed placeholder save): the stream discard awaited document.save() and ignored a false result - a refused save left the restored buffer dirty while the cleanup still skipped the dirty tab, unlinked the placeholder, and returned success, leaving the buffer one save away from recreating the denied file. The discard now treats false as a rollback failure: it stops before the close and the delete, keeps placeholder ownership so the caller skips the provider reset, and reports the hazard - the same discipline the denial-path rollback already applies to a refused save. Regression coverage: a provider-level false-save test (restore attempted, save refused, nothing closed or deleted, ownership preserved) and the execute-level wiring test (session stays unreset, hazard user-visible); negative control red with the fix stashed. Verification: DiffViewProvider suite 112/112, WriteToFileTool suites green (197 passed, 5 skipped), ESLint max-warnings=0, tsc and Prettier clean. The description's linked-issue section now names the linked unit issue as the approved issue (the earlier "no approved upstream issue exists" sentence contradicted the closing reference and is gone).
Test Procedure
pnpm --dir src vitest run integrations/editor/__tests__/DiffViewProvider.spec.ts - 112 tests pass, including the refused-save stop (restore attempted, save refused, nothing closed or deleted, placeholder ownership preserved).
pnpm --dir src vitest run core/tools/__tests__/writeToFileTool.spec.ts core/tools/__tests__/writeToFileTool-partial-state-cleanup.spec.ts - 197 pass, 5 skipped, including the no-reset wiring for a refused save.
Negative control: revert the save-result check in discardUnapprovedStream(); exactly the false-save test reddens.
tsc --noEmit and ESLint --max-warnings=0 on the changed files: clean. Earlier rounds' negative controls are one-to-one per their sections above.

Improved recovery from interrupted file writes by restoring unapproved changes while preserving approved or saved content when later steps fail.
Cleared temporary write activity when tasks are cancelled, fail, or are removed from history.
Improved parsing-error handling so partial edits are finalized and relevant streaming errors are reported.
Improved rollback of new-file edits, including missing files, cleanup failures, and abandoned editor sessions.
Ensured new-file directories are created during execution rather than during partial streaming.
Prevented stale partial edits and listeners from affecting subsequent tasks.
WriteToFileTool now tracks partial streams per task and handles streaming, parse, cancellation, and execution failures. DiffViewProvider restores streamed buffers and cleans up owned files and directories. Failed-history cleanup clears tool state before task disposal.
Changes: Write failure handling
Layer / File(s)
Summary
Partial-stream state and recovery src/core/tools/BaseTool.ts, src/core/tools/WriteToFileTool.ts, src/core/tools/__tests__/*
BaseTool delegates parse failures to a hook. WriteToFileTool tracks task-scoped stream state, handles cancellation, and cleans up stream and parse failures. Tests cover task isolation, recovery, and cancellation.
Diff-view rollback and artifact cleanup src/integrations/editor/DiffViewProvider.ts, src/integrations/editor/__tests__/DiffViewProvider.spec.ts
DiffViewProvider tracks owned placeholders, restores streamed buffers, confirms saves, and removes owned files and created directories. Tests cover rollback, abandoned streams, save commit points, and cleanup errors.
Write execution and approval-aware cleanup src/core/tools/WriteToFileTool.ts, src/core/tools/__tests__/writeToFileTool.spec.ts
Directory creation occurs during execution. Failure cleanup discards unapproved content, removes an owned placeholder before commitment, and preserves committed content. Tests cover execution errors and direct saves.
Failed-task state teardown src/core/webview/ClineProvider.ts, src/__tests__/removeClineFromStack-delegation.spec.ts
Failed-history cleanup clears WriteToFileTool state before task disposal. Tests verify cleanup ordering and abort-listener removal.
Zoo-Code-Org/Zoo-Code#1384: Both PRs define the approved WriteToFileTool path. PR #1384 changes the default PREVENT_FOCUS_DISRUPTION behavior and selects direct save or DiffViewProvider approval. PR #1932 adds cleanup and rollback across those experiment-specific save and preview paths.
Merge Risk: Moderate · up to 96730
Several cleanup and rollback paths in the write flow can still lose approved edits or leave stale diff or partial-ask state after a failed write. Fix the retained-session cleanup and the different-path guard before merging.
Security Architecture Review
Security architecture risk:🔵 Low · up to 9b93a
The change strengthens file-write failure recovery while preserving approval checks. Remaining uncertainty concerns recovery when saving messages or restoring files fails.
Retained concerns
No architecture-level concerns identified.
Security review details
Security Blast Radius
observed — The sensitive outcomes remain task-message persistence and filesystem edits made with the extension's existing authority. Write paths can resolve outside the workspace and are represented as such in approval metadata. The changed streaming state is task-instance scoped rather than shared across providers by path.
Trust Boundaries and Controls
observed — Execution retains the ignore-access check and protected-file approval context. Both direct and editor-backed save paths follow successful askApproval. Setting isAnswered during failure finalization is distinct from the writeApproved flag and does not authorize a save.
Resilience and Maintainability Implications
observed — Checked normal provider disposal aborts tasks before draining their disposal, allowing the new abort callback to release streaming state. Failed-history cleanup covers the direct-disposal exception explicitly. Task abort and disposal reuse promises, and streaming-state deletion safely tolerates an already-removed entry.
Hardening Proposals
proposed — Connect the production missing-nativeArgs rejection path to the new teardown boundary. Its existing early exit still bypasses BaseTool.handle, so malformed model output is not universally covered by the added parse-failure cleanup.
proposed — Give failed rollback and failed terminal-message persistence explicit reconciliation ownership, retaining enough state for actionable recovery. This would strengthen existing best-effort guarantees without treating successful teardown as proof that restoration succeeded.

Full details: Linked Issues check
Explanation
The PR implements the coding requirements in issue #1937. WriteToFileTool tracks per-task partial state, performs cleanup for failed or unapproved writes, and preserves placeholder ownership when rollback or save confirmation fails. The reported tests cover the execute() error path, cleanup ordering, rejected handleError(), refused saves, and rollback failures. Issue #1937 also assigns DiffViewProvider rollback support to this unit, and the reported provider changes and tests cover that assigned scope. The reported typecheck, ESLint, Prettier, and focused test runs are clean.
Full details: Out of Scope Changes check
Explanation
The DiffViewProvider and provider-test changes are explicitly included in issue #1937 because execute() cleanup depends on them. Partial-stream state release and rollback ownership support the same cleanup invariant. BaseTool.ts and ClineProvider.ts are cumulative prior-unit changes that issue #1937 identifies as belonging to U3 and U4. No unrelated change is demonstrated by the supplied evidence.
Full details: Security Boundaries
Explanation
No changed path bypasses the security boundaries. WriteToFileTool.execute() calls validateAccess(relPath) before filesystem writes, and both write paths require askApproval() before saveDirectly() or saveChanges() (WriteToFileTool.ts:385-399, 486-500, 528-542). The new cleanup paths discard unapproved stream content and unlink only provider-owned placeholders. The retained-session guard rejects writes for a different path before reuse (WriteToFileTool.ts:361-382).
Full details: Persistence Integrity
Explanation
No concrete changed persistence defect found. The changed save path awaits TextDocument.save() and treats a false result as pre-commit failure (DiffViewProvider.ts:360-379). The execute path marks commitment only from that save callback and avoids rollback after commitment (WriteToFileTool.ts:537-542, 581-595). New rollback paths await buffer edits, saves, unlink, and directory removal, retain ownership when cleanup fails, and report the hazard (DiffViewProvider.ts:696-756, 814-846).
Full details: Regression Evidence
Explanation
The prior regression remains. cleanupFailedPartialStream() skips reset() and calls both reportRevertFailure() and reportPlaceholderStranded() only when discardUnapprovedStream() rejects while ownsPlaceholder is true (WriteToFileTool.ts:250-265). The direct cleanup tests use a structural double without ownsPlaceholder, so they cover only the reset path (writeToFileTool-partial-state-cleanup.spec.ts:181-207). The ownsPlaceholder = true tests are execute-path tests (writeToFileTool.spec.ts:1526-1595) and do not invoke either cleanup helper. The corresponding onParameterParseFailure() branch is also tested only without placeholder ownership.
Resolution
Add focused tests for both cleanupFailedPartialStream() and onParameterParseFailure() with ownsPlaceholder: true and a rejected discardUnapprovedStream(). Assert that reset() is not called, both rollback-hazard and stranded-placeholder reports occur, and the task stream state is released. Keep the existing non-owned placeholder tests for the normal rollback-failure path.
Full details: Lifecycle Resource Cleanup
Explanation
The prior cleanup race remains. The changed WriteToFileTool.execute() now runs discardUnapprovedStream() and reset() in a finally after handleError() rejects (src/core/tools/WriteToFileTool.ts:560-610). If cancellation sets Task.abort, production Task.say() rejects (src/core/task/Task.ts:2692-2707), while abortTaskOnce() independently emits TaskAborted and starts disposal; disposal can call diffViewProvider.revertChanges() (src/core/task/Task.ts:3491-3518,3632-3642). The new abort listener only removes partial-stream state (WriteToFileTool.ts:95-130); it does not serialize or claim diff-provider cleanup. Therefore, an abort during rejected handleError() can run the execute cleanup and disposal cleanup concurrently, duplicating editor, buffer, tab, or filesystem operations. The added rejection test uses a mocked handleError and does not exercise task abort or disposal (writeToFileTool.spec.ts:1814-1857).
Resolution
Add a per-task serialized diff-cleanup owner shared by WriteToFileTool.execute() and task disposal. When cancellation or disposal starts, make execute() await the existing cleanup and skip its own discard/reset, or route both paths through one idempotent cleanup promise. Serialize provider, editor, and filesystem operations before releasing partial-stream state. Add an integration test that aborts during rejected handleError() and verifies one cleanup sequence.

Correct for this unit in isolation — and it is exactly what the sibling unit U7 (#1928 p1066/u7-early-return-denial-cleanup) adds. In that branch, each of the three early returns calls the
teardown before returning:
This PR (#1932, U6) only moves the finally cleanup so it also runs when handleError() rejects, so the
state the finding describes is left behind here. The split keeps each unit to one behaviour change; U7 is
the unit that owns the early-return teardown, and the chain is merged as a set (U1 → U7). Nothing to add
in this diff without duplicating U7.

…ails
onParameterParseFailure() tore down the per-task stream state first, then reverted the diff document with the
failure logged and swallowed, then reset the diff provider. A failed revert therefore looked like a completed
teardown even though the editor can still hold the unapproved partial content - and a user save of that editor
lands a write the task never approved, with nothing in the UI saying so.
The revert now runs while the recovery state still exists, revertDiffChangesBeforeReset returns whether it
succeeded, and a failed revert is surfaced with task.say("error", ...) naming the hazard. The remaining cleanup
(reset + per-task teardown) still runs, so the task is never left half-torn-down.
Test: the seeded per-task state is observed from inside the failing revertChanges double, pinning the ordering
(state present during rollback, gone afterwards), plus the error say. Without the fix: 1 failed / 5 passed;
with it: 6 passed. eslint clean with --prune-suppressions (no suppression change); package tsc reports no
errors in the touched files.
…committed
saveChanges() now throws before its commit point when the document save is
unconfirmed, but execute() had marked the write approved before that call, so
the catch skipped the unapproved-stream discard and reset() cleared the
placeholder ownership anyway: the empty placeholder stayed on disk unowned, a
later attempt treated it as the pre-existing target, and a later denial's
rollback no longer recognized it. The execute path now distinguishes approval
from a confirmed commit - when saveChanges() fails before onCommit, the owned
empty placeholder is removed before reset() forgets it, while the dirty editor
keeps the approved content for a manual save or retry.
Adds an execute-level regression test (save resolving false: no success
result, placeholder removed before reset, buffer not discarded) and provider
unit tests for the new discardOwnedPlaceholder() release.
Persistence Integrity (Error) - third finding at the new head; fixed.
The claim verified true: execute() sets writeApproved = true before calling saveChanges(), so when the new save-confirmation gate threw before onCommit, the catch skipped the unapproved-stream discard (correctly - the content is the user's accepted edit) but reset() then cleared placeholderPath and createdDirs anyway: the empty placeholder stayed on disk unowned, a later attempt would treat it as the pre-existing target, and a later denial's rollback no longer recognized it. Approval was being conflated with a confirmed commit.
Fixed per the Resolution's first option, scoped to the placeholder: when saveChanges() fails before onCommit, the execute path now removes the owned empty placeholder (new DiffViewProvider.discardOwnedPlaceholder(), ENOENT-tolerant like the other rollback deletes) BEFORE reset() clears the ownership, while the dirty editor keeps the approved content for a manual save or retry - the created directories are deliberately left in place because that save needs them. A failed placeholder removal is logged, matching the file's other cleanup helpers.
Regression coverage as demanded: an execute-level test drives save() resolving false (saveChanges throwing before the commit callback) and verifies no success result (handleError, no write result pushed), the buffer is NOT discarded, and the placeholder removal happens before reset (order assertion). Provider unit tests cover the release itself: unlink + ownership release, no-op without ownership, ENOENT tolerance. Each new test carries a negative control (exactly one red with the corresponding source change stashed).

The false-save cleanup removes the owned placeholder before reset(), but a
removal that rejects (EPERM/EACCES) was only logged: reset() then cleared the
session anyway, forgetting the ownership and stranding the empty placeholder
unowned with no user-visible hazard and no retry path. The execute path now
keeps the ownership when the removal fails - reset() is skipped so the
provider still owns the placeholder and the diff session stays open for a
retry - and reports the hazard in the chat, the same way a failed rollback
is reported. discardOwnedPlaceholder() already preserved placeholderPath
across a rejecting unlink; a unit test pins that contract.
Persistence Integrity (Error) - fourth finding at the new head; fixed.
The claim verified true at 346ee8b: the false-save cleanup removed the owned placeholder before reset(), but a removal that rejects (EPERM/EACCES) was only logged (WriteToFileTool.ts:181-187), and the surrounding cleanup always called reset() (:518-523), which clears placeholderPath (DiffViewProvider.ts:1460-1477) - stranding the empty placeholder unowned with no user-visible hazard and no retry path.
Fixed per the Resolution: the placeholder-removal failure is now a reported rollback hazard, and the owned state is preserved until removal succeeds - the execute path skips reset() when the removal fails, so the provider still owns the placeholder and the diff session stays open for a retry, and the hazard is reported in the chat the same way a failed rollback is (reportPlaceholderStranded). discardOwnedPlaceholder() already preserved placeholderPath across a rejecting unlink; a new unit test pins that contract (mutation probe: releasing ownership before the unlink turns exactly one test red). The demanded execute-level regression test drives pre-commit save failure followed by a rejecting discardOwnedPlaceholder() and verifies the failure is surfaced (chat error naming the placeholder, plus the console line), no success result is pushed, and reset() does not run - so it cannot silently strand the placeholder as unowned (negative control: exactly one red with the source fix stashed).

535-557: Report a stranded placeholder even when no revert ran.
In the writeApproved && !writeCommitted branch, reverted stays true. The else if (strandedPlaceholder) branch therefore runs, and the report is reached. The two flags cannot both be set in one pass, because they come from exclusive branches. Skipping reset() keeps placeholderPath and the diff session, as the provider test documents.
One residual effect is out of scope for this PR: execute() and handlePartial() both reuse a non-reset editType of "create", and the retry depends on that value. Ownership is preserved, so this matches the stated intent.

revertChanges() and discardUnapprovedStream() released placeholder ownership
up front, before the awaited restore, tab cleanup and unlink: a denied
new-file edit whose rollback then failed (refused restore, EPERM unlink) left
the empty placeholder on disk unowned, and execute()'s cleanup reset the
provider over the top of it, so a later attempt mistook the debris for an
existing file with no hazard reported and no retry path. Ownership is now
released only when the unlink succeeds - a failed removal leaves the
placeholder owned, execute() skips reset() while the provider still owns it
(the same discipline as the pre-commit save-failure path), and the hazard is
reported in the chat. Also pins the two coverage gaps the review flagged:
the handlePartial boundary releases the stream state when the filesystem
probe rejects, and a rejecting hazard report is logged without aborting the
teardown.
Persistence Integrity (Error) - fifth finding at the new head; fixed. Regression Evidence (Warning) - both named coverage gaps closed.
The claim verified true at e61e004: revertChanges() snapshotted placeholderPath and cleared it before any await (src/integrations/editor/DiffViewProvider.ts:727-728), so a denied new-file edit whose rollback then failed - refused restore (:758-767) or EPERM unlink (:780-782) - left the empty placeholder on disk with ownership already forgotten; execute()'s cleanup ran over the top of it and reset() cleared the session, stranding the file unowned with no hazard reported and no retry path.
Fixed per the Resolution: ownership is released only when the artifact removal succeeds. revertChanges() (and discardUnapprovedStream(), which had the same up-front clear) now keep placeholderPath set until removeCreatedFile lands, releasing it immediately after the unlink so a later directory failure cannot strand an ownership whose file is gone; the created-directory state likewise survives a failed rollback. execute() checks the provider's new ownsPlaceholder signal in its finally and skips reset() while the placeholder remains owned - the same discipline as the pre-commit save-failure path - and reports the hazard in the chat. The demanded execute-level regression test drives a denied new-file edit whose rollback and discard both reject EPERM and verifies the placeholder stays owned, reset is not called, and the hazard is user-visible (negative control: exactly one red with the tool fix stashed). Three provider tests pin the ownership contract (failed unlink keeps it, successful unlink releases it before the directory loop, discard keeps it too; negative controls via source stash and a release-ordering mutation, each exactly one red).
Regression Evidence (Warning): both omitted negative paths now have focused tests - a filesystem-probe rejection during a stabilized partial delta releases the stream state and abort listener and still reaches handleError (negative control: removing the boundary release turns exactly one test red), and the stranded-placeholder test variant rejects task.say() and verifies the failure is logged, handleError still ran, and the session stays unreset (negative control: dropping the report's catch turns exactly one test red).

…ure in discardUnapprovedStream
document.save() can resolve false without throwing. The discard ignored
the result, so a refused save left the restored buffer dirty while the
cleanup still skipped the dirty tab, unlinked the placeholder, and
returned success - the buffer was then one save away from recreating
the denied file. The discard now stops before the close and the delete,
keeping placeholder ownership so the caller skips reset() and the
session stays retryable, and reports the hazard.
Regression tests: provider-level false-save stop (no close, no unlink,
ownership preserved) and the execute-level no-reset wiring.

Cleanup of an unapproved partial write must call discardUnapprovedStream() before reset(). cleanupFailedPartialStream() performs this sequence and reports a warning if discard fails.
…etained; keep placeholder ownership through partial cleanup
A refused rollback keeps the session (isEditing + relPath + dirty buffer) so its content survives a retry. A later write for another path reused it: it skipped open(), then updated and saved the old document while asking approval for the new one. DiffViewProvider now exposes isEditingDifferentPath/editingRelPath and WriteToFileTool rejects such a write before any cleanup can reset the session, telling the user which unfinished write must be resolved.
cleanupFailedPartialStream and onParameterParseFailure now apply the same ownership guard as execute(): when the rollback still owns the placeholder, reset() is skipped (it would forget the ownership and strand the empty file) and the stranded-placeholder hazard is reported. discardUnapprovedStream restores the created-directory snapshot when a refused save stops the rollback before the artifact sweep, so a later retry can still remove those directories.
Tests: a different-path write is rejected without touching the retained session, and the refused-rollback test now asserts both hazards (restore failure and stranded placeholder) separately.

…retained-session remedy
handlePartial() streams a different path's deltas into the retained document (isEditing is true, so open() is skipped and update() writes the new path's content into the old document). It now leaves the preview untouched when the session belongs to another path and lets execute() report the rejection.
The rejection's remedy named a user action that does not clear the retained session in-process (only a retry of the retained path reaches reset()), so it looped to the mistake limit; it now tells the model to retry the retained write first. That early return also releases the per-task stream state, which every other early return does, so no TaskAborted listener or streamFailed flag is left behind.
Tests: a different path's partial delta never opens or updates the retained diff; the rejection message carries the retry-the-retained-path remedy.

The sole merge target of the series is the FINAL integration PR (1928); merging the chain in order keeps every bot-visible diff clean.Verification (this unit, as pushed)

If the bot stalls on a pre-merge check and the existing head cannot obtain bot review/approval (empty-commit re-trigger attempted and failed), the unit is recreated from the tagged content source of record — never from a per-PR head. At most 1 PR per issue.

Closes #1937 (unit U6 of the upstream PR 1066 split).
Round update — Lifecycle Resource Cleanup
Shared root cause behind the
Lifecycle Resource Cleanuprow (all five units of 1066).handlePartial()registers this task's partial-stream entry — and itsTaskAbortedlistener — before it checks the prevent-focus-disruption experiment. With the experiment enabled the delta returns without ever showing a preview and never reachesexecute()'s teardown, so the entry and the listener stay attached for the rest of the task's life, and astreamFailedmark armed by an earlier failed delta keeps suppressing this task's later diff previews. The sibling units carry the same release in their own PRs, each verified red-first with a negative control. This unit (U6):
execute()is already covered by thefinally { this.resetTaskPartialState(task) }block, so only the suppressed-preview return inhandlePartial()lacked a release; it now has one.Red first: the new test failed with
expected 1 to be +0. Green: 57 passed / 5 skipped. Negative control: removing the release turns exactly that one test red; restored green.Main refresh. Merged org main
036245c5e(U1 1927). U1's content no longer appears in this diff: 10 files +2048/−45 → 7 files +1593/−40, 0 behind main. Conflicts were confined tosrc/core/task/__tests__/Task.spec.ts(andTask.tson U7) — the region U1 rewrote; resolved by taking main's version of the shared save-stage tests (try/finally plus the fixed-task-idui_messages.jsoncleanup fromcf9206a42) rather than re-implementing U1.Verification after the merge: Task.spec 172 passed, writeToFileTool.spec 57/5, eslint 0/0.
Scope declared in the split plan
CodeRabbit's Out of Scope Changes row asks to either narrow this PR to the
execute()cleanup scope or update the linked split. The plan was updated first, per this chain's planned-and-issued rule - see, comment 6076465965 (2026-10-09), which declares U6's scope at headc6b77fbce. The files outside theexecute()cleanup scope are therefore planned, not drift:src/core/tools/BaseTool.ts- partial-stream state ownership: the per-tasktaskPartialStreamStatemap and its release pair live inBaseTool, so the prevent-focus-disruption release cannot be expressed without touching it.src/core/webview/ClineProvider.ts+src/__tests__/removeClineFromStack-delegation.spec.ts- teardown wiring: the shared finalization and the diff-view reset the cleanup path calls are owned there.handlePartial()- it registers the per-task entry and theTaskAbortedlistener; releasing that registration is the defect this unit fixes.Chain merge order: U1 1927 (merged) -> U2 1928 -> U3 1931 -> U4 1929 -> U5 1930 -> U6 1932. The same root cause is fixed once and ported; every port commit cites the source commit.
Rows from the 2026-10-09 review
Two rows in that review need files outside the
execute()cleanup set. The scope was extended in the split plan before the fix was pushed: (retired fork tracking item 41).DiffViewProvider.revertChanges()must roll the created file and directories back even whenopen()failed beforeactiveDiffEditorwas assigned. Same root cause as 1930; ported verbatim from7b783452c/6cae369d9.handlePartial()re-checks that this task's stream entry is still live after each await (provider state, filesystem probe, partial ask,open()).execute()test, same shape as the three on 1929.Fix commit:
443993e57. Negative controls are one-to-one (each re-check and each release site kills exactly its own test).Split-unit issue reference
Stryker directive sweep: removed all three
// Stryker disable next-linecomments inWriteToFileTool.tsper maintainer directive (mutation-diff is advisory; suppression comments are no longer accepted). Equivalence rationale preserved here:ConditionalExpression(path-stabilization check): the!== undefinedclause is redundant - whenlastSeenPartialPathis undefined the second clause only matches an undefinedpartialPath, which the!!partialPathin the return value rejects either way.AssignmentExpression(commit-point rollback stand-down): the approval flag already stands the rollback down, so removing the assignment changes no observable behaviour today; it stays because this branch lands the write without a diff view and the commit point is the fact that says so.LogicalOperator(rollback guard):writeCommittedis the second, independent stand-down - every path that sets it has already setwriteApproved, so no test can tell&&from||here; it stays because it names the durable fact the provider signals at the save, not the approval fact, and it is the guard that survives if the approval flag moves.CR round (Data-Integrity MAJOR, refused-restore save):
restorePreStreamBuffer()ignored the boolean fromvscode.workspace.applyEdit; a refused edit left the streamed content in the buffer andsaveBufferClean()persisted it - unapproved output on disk, worse when the close also failed becausediscardFileTab()then throws beforeremoveCreatedFile(). The restore now reports whether it applied, the save is gated on it (mirroringdiscardUnapprovedStream()), the close-failure message distinguishes an un-restored buffer, and the misleading comments are corrected. New focused test mocksapplyEditresolving false and asserts no save plus the filesystem rollback still running (negative control: exactly one red with the fix stashed). Verification: DiffViewProvider suite 99/99, ESLint--max-warnings=0,tsc --noEmitand Prettier clean.CR round 3 (Data-Integrity MAJOR, close over unconfirmed buffer): the refused-restore guard fixed the save but not the close. Both failure modes now stop the rollback and report it: a refused restore throws before
tabGroups.close()(closing the dirty tab would trade the rollback report for a save-confirmation prompt over unapproved content), andsaveBufferClean()returnsdocument.save()'s confirmation - an unconfirmed clean state stops the close the same way. The close-failure message no longer needs the restored/un-restored branch. Tests: refused restore -> no close + rollback-failure report;save()resolving false -> no close + report (negative controls: exactly one red each with the fix stashed; the existing refused-close test's save double now resolves true, matching the confirmed-save precondition). Verification: DiffViewProvider suite 100/100, ESLint--max-warnings=0,tsc --noEmitand Prettier clean.Lint follow-up: upstream main re-enabled
@typescript-eslint/no-unused-varsforsrc, which exposed dead code in this branch on the merge ref. Cleared: the unusedstateForhelper inwriteToFileTool-partial-state-cleanup.spec.ts, and the defaultonParameterParseFailureimplementation's three ignored parameters inBaseTool.tsnow carry the rule's_prefix (JSDoc updated to match). Focused suites re-run green; ESLint--max-warnings=0,tsc --noEmitand Prettier clean.Verification at this commit: writeToFileTool suites 79 passed / 5 skipped;
tsc --noEmitclean; ESLint--max-warnings=0clean; Prettier clean (comment-only diff).Updated this round (Persistence Integrity check): the walkthrough at the previous head reported that reset() awaited the diff-view close before clearing the session with no finally, so a rejected close left relPath, editType, isEditing and the placeholder ownership behind; the next execution then skipped open(), reused the stale create session, and a later denial's rollback unlinked the path derived from the stale relPath - the file the previous write had approved.
Updated this round (main refresh): merged upstream main (
8018cba40) into the branch; the merge itself was clean. The merge-ref compile job then flagged six test doubles this branch added to the write-tool spec: main madetoolCallIda required member ofToolCallbacks, and those literals predate the change. Added the missingtoolCallIdto them, matching the value main's own doubles in the same file use. Verification at this merge: typecheck clean; write-tool spec 68 passed / 5 skipped; DiffViewProvider spec 101 passed; ESLint max-warnings=0 clean; Prettier clean.Updated this round (re-review follow-ups): three review threads addressed.
Updated this round (Persistence Integrity check, commit path; Regression Evidence warning):
saveChanges()awaited the document save without checking its boolean result, so a save resolvingfalsestill crossed the commit point -onCommitran and placeholder ownership was released although nothing had persisted. The commit point now requires a confirmed save; an unconfirmed one throws beforeonCommit, leaving the uncommitted cleanup state (placeholder still owned) for the execute error path, which already handles a pre-commit saveChanges rejection without reporting success. Both coverage gaps named by the warning are closed: a focusedopen()test rejects the placeholder write and asserts ownership stays unset while the rollback cleans created directories without unlinking the unowned path, and a clean-documentsaveChanges()test asserts the commit callback runs and ownership is released without a save. Each new test carries a negative control (exactly one red under the corresponding mutation). Suite 105 passed; typecheck, ESLint max-warnings=0 and Prettier clean.Updated this round (Persistence Integrity check, execute path): the execute path conflated approval with a confirmed commit -
writeApprovedwas set before saveChanges(), so a save-confirmation failure beforeonCommitskipped the discard and reset() cleared the placeholder ownership anyway, stranding the empty placeholder on disk unowned where a later attempt would mistake it for the pre-existing target. The execute path now distinguishes the two: when saveChanges() fails beforeonCommit, the owned empty placeholder is removed before reset() forgets the ownership (ENOENT-tolerant release), while the dirty editor keeps the approved content for a manual save or retry and the created directories stay in place for that save. New execute-level regression test (save resolving false: no success result, placeholder removed before reset, buffer not discarded) plus provider unit tests for the release, each with a negative control. Suites 176 passed; typecheck, ESLint max-warnings=0 and Prettier clean.Updated this round (Persistence Integrity check, placeholder-removal failure): a placeholder removal that rejects (EPERM/EACCES) was logged only, and the cleanup then reset the session anyway - forgetting the ownership and stranding the empty placeholder unowned with no user-visible hazard and no retry path.
Updated this round (Persistence Integrity check, unconfirmed placeholder save): the stream discard awaited
document.save()and ignored a false result - a refused save left the restored buffer dirty while the cleanup still skipped the dirty tab, unlinked the placeholder, and returned success, leaving the buffer one save away from recreating the denied file. The description's linked-issue section now names the linked unit issue as the approved issue (the earlier "no approved upstream issue exists" sentence contradicted the closing reference and is gone).Test Procedure
pnpm --dir src vitest run integrations/editor/__tests__/DiffViewProvider.spec.ts- 112 tests pass, including the refused-save stop (restore attempted, save refused, nothing closed or deleted, placeholder ownership preserved).pnpm --dir src vitest run core/tools/__tests__/writeToFileTool.spec.ts core/tools/__tests__/writeToFileTool-partial-state-cleanup.spec.ts- 197 pass, 5 skipped, including the no-reset wiring for a refused save.discardUnapprovedStream(); exactly the false-save test reddens.tsc --noEmitand ESLint--max-warnings=0on the changed files: clean.
