Document the Guardrails service error behavior and add a changelog entry. The docs distinguish moderation blocks from prepaid Workers AI credit errors and Guardrails service failures across provider-specific endpoints and the
/runAPI.pnpm run format,pnpm run check, andpnpm run buildpass.
