Remove fixed tool-call caps from Codex reviews - #1

Conversation
Let reviewers use focused follow-up reads to finish verification rather than stopping at fixed tool-call limits. Keep batched reads, read-only restrictions, independent passes, and explicit coverage-gap reporting.

Issue
The Codex review procedure limits each reviewer to six tool calls and parent verification to three commands. These fixed caps can stop source verification before coverage gaps are closed.
Fix
Port the local review instruction change: retain batched reads, focused follow-ups, and explicit gap reporting while removing the fixed caps. Preserve read-only restrictions and independent review passes. Bump the plugin version to 1.0.1.
