Report concrete maintenance risks in Claude reviews - #2

Conversation
Include source-backed maintenance risks even when the current code is safe, and distinguish related existing defects from new regressions. Require exact claim and context checks before reporting a finding. Keep three independent reviewers, prohibit nested delegation and timer waits, and bump the plugin version to 1.0.2.

Issue
The Claude review procedure excludes currently safe maintenance concerns, which suppresses concrete risks supported by duplicated logic, state coupling, or API commitments. Overly literal contract checks can also produce unsupported findings about comments, shutdown ordering, and third-party attribution.
Fix
Admit source-backed maintenance risks at proportionate severity and label directly related pre-existing defects separately. Require exact claim and context checks, preserve three independent reviewers, and prohibit nested delegation and timer-based waits. Update the skill description and README, and bump the plugin version to 1.0.2 while preserving the current Codex procedure.
