Start each Codex reviewer with its issue-finding brief, then ask for one deeper look tailored to its role. Reuse the same session and evidence so the follow-up needs no repeated scan or intermediate finding payloads. Keep independent passes and parent verification, and synchronize the plugin version at 1.0.6.
Avoid echoing the role and pinned revisions already present in the reviewer's command. Keep the follow-up prompt as plain text in the same session, without another structured handoff.

Reviewers need one further look for missed issues without delaying initial issue finding or repeating their initial scan.
Fix
Start each Codex role with an explicit issue-finding brief, then issue one role-specific deeper-look prompt in the same native agent session. Reuse inspected evidence and parent verification. The driver prints only the follow-up instructions, with no intermediate observations, finding-collection phase, or extra agent. Synchronize the plugin version at 1.0.6.
