Persist agreed PR Decisions across independent runs - #41

Allow hosts to share the complete Decisions cache through an exclusive DK_PR_DECISIONS_HOME root while preserving the existing default location. Serialize each modifying PR operation and synchronize atomic replacements. Retain exact agreement evidence, replacement chains and uncertain publication receipts across disposable-run cleanup and source changes. Record agreed choices immediately in review and iteration procedures. Document recovery and migration, preserve temporary imports, and cover independent processes, concurrent changes, interrupted writes and preparation invalidation. Synchronize plugin versions at 1.3.0. Closes #34.

Issue
Agreed PR review choices disappear when independent runs use disposable data directories. Later reviews can repeat settled questions, and concurrent refreshes can overwrite locally recorded choices (#34).

Fix
Add DK_PR_DECISIONS_HOME as the exclusive absolute root of the complete Decisions cache, scoped by GitHub host, base repository and PR. Keep the current location when unset and reject invalid or repository-local stores without fallback. Code Factory supplies persistent storage and access separately.
