Move a Mini App into its own browser tab

An open Mini App can be moved into a browser tab of its own that shows only the Mini App, and the client
knows which presentation is live.

The inline tab offers opening the Mini App in its own browser tab, and that tab renders only the
Mini App view.
The inline view reports that the Mini App is open elsewhere and offers switching to it rather than
opening a second tab.
Closing that browser tab releases the presentation, and the inline view stops reporting it.
A deep link naming a session that has ended still shows the Mini App and reports the session as
ended.
The contract's detached flag and presentation revision reflect the live presentation rather than a
constant.
The standalone view offers no in-page control that closes the tab or returns to the inline
presentation.
