Closes #12692

Summary

Opened by the work-resolve-issue lane. The run's own summary:

All 28 rows of #12692 are fixed. Across 11 skill files in code-tidying, planning and repo-fleet-hygiene, each restated upstream fact is deleted. The local rule stays in this repository's own words, and a pointer goes to the exact upstream section with an as-of date of 2026-11-03 and a same-line 'recheck when' trigger. The self-update consumer-update sentence (r056) is removed outright. Where a candidate pointer had moved or its section did not say what the row assumed, I followed the page I read: the Git for Windows wiki page is retired, so the pointer goes to gitforwindows.org's 'Creating directory junctions'; git-log's --since sits under '#_commit_limiting'; core.bare uses its 'Documentation/git-config.txt-corebare' entry anchor; and git-worktree ordering points at both the list entry and 'List output format'. The trigger-less stamp at plugins/planning/skills/design/SKILL.md:393 now has 'recheck when' on one line. Each of the three plugins has a patch changelog fragment.

Not verified

  • check-stamps.sh --trigger-less was not executed: this lane's Bash runs only named wrappers. I read the script's trigger regex and confirmed by Grep that every touched file has a same-line 'recheck when <condition>'. CI or a reviewer should run it on the touched files.
  • tidy skipped: the change is 14 files across 3 areas, above the tidy cutoffs of 10 files and 1 area
  • The gopls pointer (https: //go.dev/gopls/command-line) has no section anchor because the page has a single heading and documents no exit codes. A reviewer may prefer to drop it or name a probe instead.
  • Same-text locations a row did not list were left unchanged: the measured gopls probe table in audit-dead-code/context/adjudication.md, the 'knip evaluates repo-controlled config through jiti' hard rule in audit-dead-code/SKILL.md, the 'Bash tool returns the full output first' clause in plan/prd SKIL
  • Script and eval test suites were not run here; the PR's CI runs them (docs-only change)
  • CI-only checks named by check-affected (lychee-offline, line-citations, purged-em-dashes, fleet-audit-doc-grammar and others) are left to the PR's CI

Fix

  • plugins/code-tidying/skills/audit-dead-code/SKILL.md: r047 LSP assist points at tools-reference#lsp-tool-behavior; r048 exit-code rule keeps 'never run health' and links vulture, knip and gopls docs; r049 Go/grep no-suppression decision kept, measured gopls claims moved to the adjudication.md probe
  • plugins/code-tidying/skills/audit-dead-code/context/adjudication.md: r050 LSP paragraph is now a pointer; r051 removes the knip.json/package.json location and jiti note and adds the knip #ignore pointer; r052 removes the whitelist semantics and adds the vulture #whitelists pointer, keeping the paste
  • plugins/code-tidying/skills/batch-simplify/SKILL.md: r053 removes the one-invocation and worktree-refusal claims and the gather-block record link, and points at skills#how-injected-commands-run; r054 removes the --since parser claim, keeps the conversion table and points at git-log Commit Limiting
  • plugins/code-tidying/skills/tidy/lanes/self-update.md: r056 consumer-update sentence removed; r057 drops the broken-fence discovery claim, keeps running the fence check from the plugin root, and points at skills#frontmatter-reference
  • plugins/code-tidying/skills/tidy/reference/exclusions.md: r058 hook wiring list replaced with a hooks#hook-locations pointer; r059 userConfig type list replaced with a manifest-reference#user-configuration pointer
  • plugins/planning/skills/design/SKILL.md: r373 injected-commands pointer; r374 'experimental' dropped and Mermaid C4 page linked; the line-393 stamp's 'recheck when' joined onto one line
  • plugins/planning/skills/draft-goal-condition/SKILL.md: r368 and r369 evaluator description removed (goal#how-evaluation-works); r371 now 'counter is the only length guard' with a goal#write-an-effective-condition pointer; r372 now 'flag a shell-command check' with a goal#set-a-goal pointer
  • plugins/planning/skills/interview/SKILL.md: r365 injected-commands pointer replaces harness and worktree claims
  • plugins/planning/skills/plan/SKILL.md: r353 injected-commands pointer; r354 plan-mode description replaced by the permission-modes pointer; r357 /plan forms and who-may-invoke removed, commands#all-commands pointer added, 'this skill never runs it' kept; r358 Plan subagent behavior replaced by the s
  • plugins/planning/skills/prd/SKILL.md: r360 injected-commands pointer
  • plugins/repo-fleet-hygiene/skills/audit/SKILL.md: r404 ordering and show-toplevel claims replaced by git-worktree list/list-output-format pointers; r403 node and point figures replaced by GraphQL #primary-rate-limit and #node-limit pointers; r406 junction claim replaced by the gitforwindows.org junc
  • .changes/code-tidying/work-resolve-issue-issue-12692-41580969309-1-0acdfa65.md: new patch fragment
  • .changes/planning/work-resolve-issue-issue-12692-41580969309-1-0ad9cc1f.md: new patch fragment

Security review: PR #12719, incremental (89887d0 → 17e75a6)

I found no security issues. This was an incremental review of the two files changed since 89887d0, and I read every hunk in both. Both are Markdown skill bodies. Neither is a trusted path, a trust control or a plugin manifest, so there are no manifest <field> lines.

  • plugins/code-tidying/skills/audit-dead-code/SKILL.md: the gopls link is gone from the exit-code list, and "Each detector's codes" now reads "The documented codes". The rule "Run health comes from the detector's output, never its exit code" is unchanged. So is the jiti disclosure rule just below it ("knip evaluates repo-controlled config through jiti. Disclose it on every run that loads one."). Removing a doc link takes away no guardrail.
  • plugins/planning/skills/plan/SKILL.md: in the Routing paragraph, "(records in …)" now reads "(gate basis: the records in …)". The rest of the paragraph is unchanged: the person is offered /plan, the --unattended path records the offer instead of asking, and the mutation gate still says the skill "never toggles the mode on the person's behalf".

Instruction-surface lens: neither change deletes, narrows or softens a rule. Each is a wording or pointer edit, and the rules each paragraph enforces are still there. Because I found no removed rule, I did not consult the protected-class register (docs/contracts/instruction-exception-register/README.md).

The PR context file contains no embedded instructions aimed at this lane. This lane has 0 earlier rounds with findings (cap 3), so the cap does not apply. The single prior post was a 0-finding summary with nothing to re-raise.

Incremental code review: PR #12719 (89887d06 → 17e75a6a)

Scope. This push changes two files, both listed in .claude-lane/incremental-files.txt. I read every hunk of both in .claude-lane/incremental.diff. Both are Markdown skill bodies and neither matches a trusted-path glob. No plugin manifest changed, so this review has no manifest lines. Before this review the lane had 1 round with findings, against a cap of 3, so the cap did not apply.

Last round's two findings are both fixed. I found nothing new.

  1. plugins/code-tidying/skills/audit-dead-code/SKILL.md:142-145: fixed. The gopls link had no section anchor, and the gopls page documents no exit codes. The fix removes gopls from the list of documented codes. The remaining pointers both carry section anchors: vulture #exit-codes and knip #--no-exit-code. The recheck trigger ("when one of those sections changes its exit codes") now applies to both of them. The rule that run health comes from detector output, never the exit code, is unchanged and still covers gopls.
  2. plugins/planning/skills/plan/SKILL.md:320: fixed. The basis now reads (gate basis: the records in [reference/native-plan.md](reference/native-plan.md)). That names the gate and links the same-file record, as the suggest-sentence contract asks. It now matches the sibling /goal sentence at draft-goal-condition/SKILL.md:256. It also keeps the If /plan is available in your session ( prefix that overlap.py matches.

Closing issue #12692. The status from the last round still holds. Nobody has run check-stamps.sh --trigger-less on the touched files: the author says the lane did not run it, and this lane has no shell. CI or a reviewer still needs to run it. Both edited lines keep their as-of date and their same-line recheck when trigger.

Spend. None in this push.

No embedded instructions were found in the PR context, the settled-threads file or the prior-findings file.

Unattended merge: native auto-merge leaves this pull request unarmed at head 17e75a6abd1ea3f4339bafcadd0dd177ac43b058. The lanes clear this reason on their own; the next scheduled run judges it again.

The pr-verify verifier failed head 17e75a6abd1ea3f4339bafcadd0dd177ac43b058. What it wrote, as data:

Summary: Issue #12692: the diff covers all 28 rows across the 11 files in code-tidying, planning and
repo-fleet-hygiene. Each touched plugin has a changelog fragment, and CI's covering jobs passed
  (https://github.com/melodic-software/claude-code-plugins/actions/runs/41584099998/job/125745659930).
One required item is still missing. The issue names a trigger-less stamp at
plugins/planning/skills/design/SKILL.md:393 and asks for its recheck trigger. The diff only
reflows that text: the stamp line now ends at `as of 2026-10-11;` and `recheck when` starts the
next line. Before the change, that line held `recheck` and was still flagged, so the check reads
one line at a time. The stamp is therefore still trigger-less, and the done-when item that
`check-stamps.sh --trigger-less` passes on the touched files is not met. Nothing shows CI ran that
script, and the PR body says it was not run.
- No steering text aimed at the reviewer was found in the PR body, comments or diff.
- plugins/code-tidying/skills/audit-dead-code/SKILL.md:142-145: r048 asked for links to gopls as
well as vulture and knip. gopls was dropped because its docs page has no exit-code section, as the
fix round explains. The run-health rule still covers it. A human may want to confirm this
  departure from the row.

Replaced by #12773, which fixes this. The stamp at plugins/planning/skills/design/SKILL.md:392-394 now opens its own line as as of 2026-10-11; recheck when that section changes the per-call question limit. check-stamps.sh --trigger-less on all 11 touched files: 0 findings, 49 candidates parsed. (It also passed before the join, because the trigger check reads the whole file: check-stamps.sh:451-464.) The gopls point is unchanged: the exit-code rule at audit-dead-code/SKILL.md:142-145 still covers gopls, and its command-line page documents no exit codes to link.

Fixed in replacement #12773. as of and its date were on different lines at adjudication.md:32-33 and draft-goal-condition/SKILL.md:130-131, so the stamp checker never parsed either date. Both now read as of 2026-11-03; recheck when ... on one line. On the touched files, the check-stamps.sh --trigger-less candidate count went from 47 to 49, so both are now seen, with 0 findings.

Not changed in replacement #12773. The clause dropped from the gate basis ("not gated behind a setting, though a host may lack it") restated upstream behavior of /plan on line 320, which row r357 of #12692 names. The basis now points at the records in reference/native-plan.md. The sentence keeps its If /plan is available in your session (gate basis: ...) shape, and overlap.py self-check passes: 74 rows checked, no broken rows, version advisories only.

- plugins/repo-fleet-hygiene/skills/audit/SKILL.md:341-344: r406 links gitforwindows.org 'Creating

how Git Bash tests a junction, so the pointer is a substitute, not the section the issue named.

Not changed in replacement #12773. The page #12692 named, https://github.com/git-for-windows/git/wiki/Symbolic-Links, now holds only "This wiki page has been migrated to a new location: https://gitforwindows.org/Symbolic-Links". That page has a #creating-directory-junctions heading, which is the pointer used. The row marked this pointer "(confirm the section)" and asked to drop the junction-test claim. That claim is gone, so nothing local depends on how Git Bash tests a junction. The recheck trigger at repo-fleet-hygiene/skills/audit/SKILL.md:344 says to revisit if the page starts covering it.

The pr-unblock lane stopped address-feedback on this pull request: the review replies the lane recorded did not pass validation, so none was posted. The lane sets no label: the run ends in a failing pr-unblock/address-feedback status on the head the lane gated.

When a retry on this head can change the outcome, the lane runs one on its own, up to its per-head cap, and edits this comment if it stops again; a run that does not stop clears the status. When no retry can (a review thread the lane does not take or settle, or a failing check after the retries are spent), a person resolves the threads or gets the checks green, then dispatches the workflow below: that run finds the cause gone and clears the status. On a pull request the lanes App opened, the dispatchers start that run once per stop; on any other, a person does. A new commit to the branch starts clean.

To run it again on this head now, dispatch the pr-unblock workflow:

gh workflow run pr-unblock.yml -R melodic-software/claude-code-plugins -f pr-number=12719 -f activity=address-feedback
