feat(tools): fail any upstream template scoring under 95 (#2106) - #2122

score_templates --check now holds every upstream template to 95, with or without a baseline row; a fork's templates keep their rows and the new-file floor. A planted case covers the floor, a fork's file and a rounding drift. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>

The score check now holds every upstream template to 95. The score PRs #2107–#2121 brought each template there; this gate keeps them there.
tools/score_templates.py--checkfails an upstream template under 95, row or no rowtests/baselines/template-quality.txtdocs/playbook.mdChangelog: no entry. The floor applies to upstream templates only, which a fork never edits, so no consuming project sees a change.
