fix: resolve the dev manifest bridge after early imports (#416, conflicts resolved) - #463

Look up the bridge origin when an isolated runner requests assets instead of permanently selecting the JS-only fallback during early startup. Keep manifest imports independent of the HTTP listening event so hosts can await them from environment startup without deadlocking. Add regression coverage for eager imports, occupied ports, early asset lookups, shutdown, in-process resolution and middleware mode. Fixes #414

This is @everton-dgn's #416 (fixes #414) carried onto current
next. #417 landed first and conflicted with it. The author's commits are unchanged; there's one merge commit on top that resolves three files:src/index.ts, bridge resolver: keeps both guards.
