feat(tools): publish apply_patch through the guard (U6, #1375) - #1915
This PR routes the apply_patch tool through the guard, so a patch hunk can no longer "write" a file the model never fully read: an unobserved overwrite and a stale version token are refused with the standard remediation. A move carries the source's completeness to its destination instead of claiming completeness for lines the model never read, and a partial-source move onto an already-observed destination is rejected before any state changes. It also fixes a resource leak: when the guard refuses a publish, saveDirectly now rolls back the parent directories it created.
It is the sixth of nine PRs for epic #1375 (chain tracked on #1991).
apply_patch publishes via guardedWrite, so an unobserved overwrite and a stale version token are rejected with the read-first / re-read-then-retry remediation instead of clobbering the file.
A move re-targets the source's observation onto the destination: the destination inherits the source's complete flag, so a slice, range, truncated, or indentation-block source never authorizes a full-file replacement at the new path.
A partial-source move onto a destination that was already observed completely is rejected before any file or registry state is touched.
saveDirectly captures the list createDirectoriesForFile returns and, if the guard rejects, removes those directories innermost-first with rmdir (which refuses a directory another writer populated, so the loop stops at the first failure) and rethrows the original write error.
Ancestor-identity pinning at publish: the confined write records the identities of the directories it created and re-checks them before the commit rename, refusing with AncestorReplacedError if an ancestor was swapped underneath it.
Why the diff looks bigger than the change: the chain is non-cumulative - this branch is based on main and carries its own copy of the earlier units' content, which the GitHub diff shows as context.
Review focus:src/core/tools/ApplyPatchTool.ts, the guard wiring in src/core/tools/guardedWrite.ts, the rollback path in src/integrations/editor/DiffViewProvider.ts, and the specs named below.
Test Procedure
From the repository root, with dependencies installed:
Environment: Node 22+, Linux/macOS/Windows CI runners (the guard's platform-specific branches are exercised through the injected platform option, not a real Windows host).
Pre-Submission Checklist
Scope is one gate scope and matches the unit.
No .changeset or CHANGELOG changes (AGENTS.md).
src/eslint-suppressions.json unchanged - no suppression count increased.
New code lints clean (--max-warnings=0) rather than relying on suppressions.
Tests added at the lowest layer that would have caught each finding.
Visual Snapshot item considered: not applicable (no rendered surface).
Visual Snapshots
Not applicable - no UI change.
Videos (interaction / animation only)
Not applicable.
Documentation Updates
None. No user-facing setting, command, or documented behavior string changes; the guard's remediation text is already introduced in earlier units. No .changeset and no CHANGELOG edit (AGENTS.md).
mutation-diff is red on the changed-line budget only: the gate counts the GitHub diff, which carries the unlanded predecessors (1205 changed executable lines against the 500 cap; this unit's own delta is 105). The row clears when the predecessors land and this branch is rebased.
The directory-rollback test is a real pin: with the DiffViewProvider.ts change stashed it fails.
Local lanes at this head: integrations + core/tools + activate - 1340 passed / 17 skipped across 60 files; tsc --noEmit clean; eslint clean on the changed files with no suppression-count increase.
CI at this head: all required checks green; mutation-diff red on the budget-cap count described above (no surviving mutants in this unit's own lines).
Reviewer and maintainer questions are welcome as comments on this PR or on #1991.
Stryker directive removal (equivalence notes)
Per AGENTS.md the two Stryker suppression comments in the guarded save
path were removed instead of kept; their equivalence claims:
writeKind default StringLiteral ("update" -> ""): an empty kind
dispatches exactly like "update" inside guardedWrite - only "edit"
and "create" branch distinctly - and the default-parameter line is
not part of this unit's changed-line selector, so the mutant is
equivalent / out of scope.
saveTask operand of the adoption-probe guard (LogicalOperator): GuardRejectedError is only thrown by guardedWrite, which is reached
only after the !saveTask check above has already thrown. Whenever
the operand is evaluated saveTask is provably non-null, so dropping
the operand cannot change which branch is taken.
mutation-diff remains advisory for stacked units; maintainers decide any
narrow gate-config exclusion.
Committed gh device ID removed (.local/state/gh/device-id, gitignored,
swept in by an add -A during a refresh). No other .local/ strays on any
unit branch or main.
Changed-block as any cleanup in DiffViewProvider.spec.ts: bracket
access for private members + documented unknown-based structural casts for
VS Code doubles; suppression 310 -> 285.
Cleanup-safety-net test asserts content, not just count: the single console.error must carry [Original Error Caught] with the commit-failure
object as its second argument, and no call may carry the cleanup-error text.
Re-review responses (error-fix re-review wave)
ApplyPatchTool move, durability-error branch (Minor Functional): first
answered by completing the source unlink and reporting unlink failures as PartialMoveError - since SUPERSEDED by the crash-availability reversal
below; see the next section for the shipped semantics.
A follow-up review (Major Data-Integrity) reversed the previous round's
guidance for the same branch, and the new semantics shipped: when the
destination's commit rename landed but its directory entry is not known to
be durable (PostCommitDurabilityError), the source is KEPT and the
durability error propagates - unlinking it could leave NEITHER path
available after a crash. The duplicate is visible to the model and
recoverable; a file lost to the crash is neither, so crash availability
outranks not leaving the content at both paths. The try/catch is gone (the
error propagates naturally) and the durability test now asserts the source
unlink was NOT called; the unlink-failure test from the superseded round was
removed with its unreachable scenario. Negative control: restoring the
unlink reddens exactly the kept-source test.
Move deletion bound to the recorded observation (440dca4)
The Persistence Integrity row demanded the source deletion prove the
source is still the file the observation authorized. The move now
deletes under the shared advisory lock (withFileLock on the canonical
lock key): the source must still be present in the observation registry
and its recorded version must equal a fresh bigint stat of the source,
otherwise the move raises PartialMoveError before any unlink. A stat
failure is likewise a PartialMoveError - an unreadable source is not a
deletable source. Tests: the never-observed case now asserts no unlink
and no destination tracking; three new tests pin the changed-source
rejection, the vanished-source rejection, and that the deletion runs
under the source's lock key. 26/26 green; negative controls (faithful
mutants of each check) each redden exactly one test.
Durability note: on the durability-error path the source is kept
outright - PostCommitDurabilityError propagates before the deletion is
reached - the strongest form of the protected-deletion contract this
unit's issue asks for.
Security Boundaries disposition (warn-only DACL). The row asks for
the DaCl restore failure to fail closed. That change belongs to the
publish-primitive owner (PR 1916, fixed there in 3624e4f), which
merges before this unit in the declared order; this unit carries the
same carrier copy and reconciles at rebase (record 6106959976 on issue
1408; tracking 1991). Implementing it twice here would fork the fix.
Walkthrough carry-over evidence (04:34 walkthrough vs head 440dca4)
The 04:34 walkthrough still carries the Persistence Integrity and
Security Boundaries ERROR rows whose claims predate this head (pushed
2026-10-16T03:55Z). The PI row's raw-unlink line numbers belong to the
superseded head 993fc6f; at this head the deletion is lock-serialized
and observation-bound exactly as the Resolution demands (missing,
changed, vanished, or unobserved sources are kept; PartialMoveError
names the committed destination and remaining source; the durability
path keeps the source outright; tests and negative controls above). The
SB row re-flags the carrier copy's warn-only DACL restore, whose
fail-closed fix lands at the publish-primitive owner (PR 1916, 3624e4fea) that merges before this unit; copies reconcile at rebase
(record 6106959976 on issue 1408). Evidence comment posted on the pull
request; no code change this round.
File writes now verify that files have not changed since they were read, with added safeguards for partial reads.
File publishing uses staged writes, supports path confinement, and checks symbolic links and concurrent changes.
Editor saves use the same file-change checks, and previews can be opened without writing them to disk.
Bug Fixes
Read results now distinguish clipped lines from omitted lines.
Failed writes preserve their original errors and clean up temporary files where possible.
Failed moves are reported instead of being treated as successful.
Tasks now record versioned file observations and read completeness. Patch, diff, and editor writes use guarded publication. Text and JSON writes use staged publication with target checks, locking, and cleanup.

Security Architecture Review
Security architecture risk:🟡 Moderate · up to 5a0dd
The change improves protection against stale and partial-file overwrites. However, replacing an existing file can weaken Windows access restrictions when permission restoration fails. Some move operations also retain their existing non-atomic behavior.
Retained concerns
Medium · security · inferred: Existing-file direct tool writes now replace the file instead of updating it in place. On Windows, publication precedes DACL restoration, and failures saving or restoring the original DACL are swallowed. Where the replacement has broader permissions than the original, an approved content edit can therefore expose the file to additional readers or writers, temporarily or persistently. Successful restoration mitigates the persistent case but does not make access-control preservation a publication precondition.
Security review details
Security Blast Radius
inferred — The inspected exposure is host filesystem content published with the extension process's existing authority. The Windows concern affects individual rewritten files whose original DACL is more restrictive than the replacement's permissions; additional principals may gain read or write access without receiving elevated process privileges.
Security Findings and Attack Paths
inferred — A legitimate approved write to a Windows file with a restrictive explicit DACL reaches replacement publication. If saving or restoring that DACL fails, the write still succeeds. A principal permitted by the replacement's broader permissions can then read or modify content previously restricted by the original DACL. The failure behavior is demonstrated by mocked tests; deployment-specific permission widening was not reproduced.
Trust Boundaries and Controls
observed — Observation authority is task-scoped and separates content completeness from filesystem version identity. The guard checks cancellation after queueing and under the lock, rejects stale versions, and retains partial completeness after targeted edits.
observed — Publication deliberately follows existing symlink referents and rejects dangling links. Move containment is lexical, while ignore matching resolves referents but allows outside-directory paths or errors. Tool writes already followed symlinks at the base, so this is not established as a new tool escape. JSON writes now follow referents instead of replacing links; production-path attacker control remains unresolved.
Resilience and Maintainability Implications
observed — Rejected editor saves use discard-only recovery rather than writing the original preview back over newer disk content. Placeholder removal checks its captured version under the shared lock, and overlapping teardown operations are serialized. Successful publication clears an unchanged dirty buffer by reloading from disk rather than performing another unguarded save.
observed — Move destination publication and source deletion remain separate operations. Source deletion has no version check and its failure is logged rather than propagated; the non-focus move branch still uses raw destination writes. Comparison with the base confirms these lifecycle limitations predate the PR, so they are not retained as newly introduced concerns.
Hardening Proposals
proposed — Make preservation of an existing Windows DACL a publication precondition: prepare and verify equivalent restrictions on the replacement before making it visible, and reject the write when preservation cannot be established.
proposed — Document the resolved-target authorization policy for tool and persistence writes, then test outside-workspace referents and referent changes. Treat source-version-protected move cleanup and consistent guarding across execution modes as follow-up work for the existing lifecycle gaps.

Full details: Linked Issues check
Explanation
Issue #2021 requirements are met. ApplyPatchTool uses guardedWrite for update, create, and move publishes, so unobserved and stale writes receive guard rejection and remediation. Move publication carries the source complete state and rejects a partial source with an observed destination before publication. saveDirectly removes newly created directories in reverse order after guard rejection and rethrows the original error. The summary reports focused tests for these behaviors and for protected move-source deletion.
Full details: Out of Scope Changes check
Explanation
The observation registry, read-scope tracking, guarded publish path, safe-write identity checks, rollback logic, move-deletion checks, and related tests support issue #2021 or its required publish path. The larger chain content is identified as prerequisite or carrier implementation for the guarded path. No unrelated change is established, and no files were excluded from review.
Full details: Security Boundaries
Explanation
The standing Windows access-control finding remains in changed src/services/file-safety/safeWriteText.ts. For an existing target, the code captures the DACL before fs.rename, but when _restoreDaclWindows fails after the commit, lines 878-893 call warn() and return normally. The replacement can therefore retain different, potentially broader inherited access rights while the write reports success. Changed guardedWrite calls safeWriteText at lines 167 and 243, and ApplyPatchTool uses this guarded path, so an approved patch can publish without verified access-right preservation. The changed test at lines 862-891 explicitly expects successful completion after restore failure.
Resolution
Make existing-target DACL restoration fail closed. Do not report successful guardedWrite or ApplyPatchTool completion when restoration fails. If the rename must occur first, apply a restrictive fail-closed ACL to the replacement before returning, surface the restoration error, and add a regression test that verifies restore failure cannot leave a published file with broader unverified access.
Full details: Persistence Integrity
Explanation
The rollback defect remains in the changed safeWriteText path. For a new nested target, _stagingDir calls recursive mkdirSync at src/services/file-safety/safeWriteText.ts:250-255 before _missingDirectoryTail runs at lines 609-613. Therefore createdDirs is empty even though the write created the target's parent directories. If the pre-commit rename at lines 845-846 fails, cleanup removes the staging file and staging directory but removes no parent directory at lines 925-943. The operation reports failure while leaving empty parent directories as persisted filesystem state. guardedWrite awaits this publisher at lines 167 and 243, so the changed apply-patch persistence path can trigger the defect. The added test at safeWriteText.spec.ts:1727-1751 mocks the parent as missing after mkdirSync, so it does not model this real-filesystem ordering.
Resolution
Record the missing parent-directory chain before _stagingDir creates it, or make staging setup report and retain all directories it creates. Include that chain in the pre-commit cleanup, remove the staging file and staging directory first, then remove owned parent directories innermost-first. Add a real-filesystem integration test with an absent nested parent and a forced pre-commit failure, and assert that no created parent remains.
Full details: Regression Evidence
Explanation
The Task integration still lacks focused coverage. The pull request adds readonly observationRegistry = new ObservationRegistry() in src/core/task/Task.ts:286-288, but src/core/task/__tests__/Task.spec.ts is unchanged and contains no observationRegistry assertions. The new observationRegistry.spec.ts only tests standalone ObservationRegistry instances, so it does not verify that a real Task constructs the registry or that separate Task instances receive independent registries.
Resolution
Add focused tests in src/core/task/__tests__/Task.spec.ts that construct a Task, assert task.observationRegistry is an ObservationRegistry, and construct two tasks to assert their registries are distinct. Keep the standalone ObservationRegistry tests.
Full details: Lifecycle Resource Cleanup
Explanation
The changed DiffViewProvider.revertChanges() path can duplicate teardown after cancellation. It awaits runTeardown() at lines 910-959, but runTeardown() clears teardownInFlight at lines 1106-1110 before revertChanges() restores preview tabs at lines 972-974. If restorePreviewTabs() is awaiting showTextDocument() for a captured preview tab, a second cancellation or Task.disposeOnce() can enter revertChanges(), see no teardown in flight, and start another document revert, tab cleanup, and preview restoration. sessionFinalizationClaimed does not prevent this because finalization occurs only after the second teardown can begin. The added tests cover callers waiting during the teardown callback, not a caller arriving during preview restoration.
Resolution
Keep teardown ownership active through preview restoration and session finalization. Use one shared teardown/finalization promise or explicit owner state for the complete lifecycle. Make cancellation and disposal callers await that state, and clear the state only after all document cleanup, preview restoration, and finalization work completes.

Confirmed — this is the same defect as the other three fsync threads, not four separate ones.
The rollback in the catch ran whenever backup mode had renamed target -> backup, including when the failure happened after the commit rename had already published. PostCommitDurabilityError tells the caller the content is at the target path, but the catch had already renamed the backup back over that target, so the error message and the file disagreed.
Fixed in the unit that owns the publish path, #1910, commit 58f803ddb: a committed flag is set immediately after the commit rename and the rollback is skipped once it is set, so only a pre-commit failure can restore the backup.
Regression test at the lowest layer that would have failed (commit rename succeeds, post-commit directory open fails, backup mode on): fails without the guard, passes with it. This unit carries the same copy of safeWriteText.ts, so it inherits the fix once #1910 merges first in the chain order U1 -> U2 -> U3 -> U4 -> U5 -> U8 -> U6 -> U7 -> U9.
Partly valid, but outside this unit's scope. The else branch is pre-existing main behaviour: the guarded publish is gated by the PREVENT_FOCUS_DISRUPTION experiment by design, and this unit's stated scope is the guarded path. Routing the experiment-off path through guardedWrite is a behaviour change for users with the experiment off, so it is not a fix this unit can make on its own. Recorded as a follow-up on the tracking issue so the gap is not lost: with the experiment off, an apply_patch move writes the destination with plain fs.writeFile and no completeness check.

499-500: The experiment-off move still writes the destination without the guard.
The enabled path now publishes through guardedWrite(..., "create", sourceComplete). The else branch at Line 506 still writes moveAbsolutePath with plain fs.writeFile, and it runs no partial-source check. A previous review reported this issue. The author recorded it as a follow-up outside this unit's scope.
…locks
- Remove .local/state/gh/device-id: the GitHub CLI's persisted device ID
(a pseudonymous telemetry identifier) was swept in by an add -A during
a refresh round. It is gitignored; nothing else under .local/ is
tracked on this or any sibling unit (checked all six).
- DiffViewProvider.spec: the 25 added as-any casts in the changed blocks
become bracket access for private members and documented
unknown-based structural casts for the VS Code doubles (same pattern
as the sibling unit's thread); the file's no-explicit-any suppression
drops 310 -> 285, nothing else moves.
- safeWriteJson.lockKey.spec: the cleanup-safety-net test now asserts
WHAT the single console.error reported - the message carries
'[Original Error Caught]' and the second argument IS the commit
failure - and that no call carries the cleanup-error text. A
regression that logs the ENOENT cleanup instead of the commit failure
can no longer pass on the count alone. Negative control: making the
cleanup log unconditionally reddens the upgraded test (plus two
neighbors that lean on the same silence).
158/158 green across the two specs, eslint clean, prettier clean, tsc
clean.

…cess
Persistence Integrity: handleUpdateFile's move published the
destination, then removed the source with a swallowed catch. Two
failure shapes ended as a completed move with the content at BOTH
paths: (a) saveDirectly can throw PostCommitDurabilityError AFTER the
commit rename has already landed the bytes at the destination - the
error skipped the source unlink and the source survived beside the
destination; (b) a failing source unlink was logged with console.error
and the flow continued into trackFileContext and the success result.
The guarded publish now catches PostCommitDurabilityError for the
destination path, completes the observable half of the move (unlink the
source, logging if THAT also fails so both paths are named), and
rethrows - a durability failure is never reported as a completed move.
The source unlink failure propagates as a dedicated PartialMoveError
naming both paths, so edit tracking and the tool result no longer claim
a move that left the file twice.
Two regression tests: landed-but-not-durable publish still removes the
source and reports the durability failure; unlink failure reports
PartialMoveError and skips moved-file edit tracking. NCs: removing the
post-commit handling reddens exactly the first; restoring the swallowed
catch reddens exactly the second. 29/29 apply-patch specs,
eslint/prettier clean, tsc adds nothing beyond the known local donor
drift.
main's tool-call scoping (Zoo-Code-Org#2015) made ToolCallbacks.toolCallId required;
the unit's own spec callbacks name it with the repo's test convention so
check-types passes on the merge ref with either BaseTool shape. The
delete-path literal is left untouched - main's own addition covers it.

126-135: Fix the comments that contradict the completeness rule.
Lines 134-135 record complete: false when no prior observation exists. That behavior is correct. The comments at Lines 128-129 and Lines 131-133 still say that a read with no prior observation "is a complete observation." A maintainer who follows these comments can reintroduce the full-file authority defect.
♻️ Proposed fix
- // The tool's own hunk read, not a model read. When the model already observed the- // file, keep the completeness it earned and only on the version it was earned on; a- // partial view stays partial. With no prior observation this read returned the whole- // content, so the observation is complete.+ // The tool's own hunk read, not a model read: it authorizes the targeted edit+ // only. Completeness carries over only from a prior complete model read of this+ // same version; otherwise the observation is partial.
const prior = task.observationRegistry.get(absolutePath)
- // Nothing to carry when the model never observed the file: this read returned the- // whole content, so it is a complete observation. Carry only when a prior observation- // exists and still describes the version that was read.
const complete =
- prior === undefined ? false : prior.complete === true && prior.version === preReadToken+ prior !== undefined && prior.complete === true && prior.version === preReadToken
Based on learnings: "Record a partial observation when no prior observation exists, and preserve completeness only from a prior complete observation with the same version token."
…r unlink fails
A PostCommitDurabilityError means the destination landed; the handler then
removes the source. When that unlink fails too, the content exists at BOTH
paths while the model only ever saw the durability failure - the state the
dedicated PartialMoveError class exists to report (same contract as the
success-path unlink). The durability error is replaced by the strictly
worse half-applied state, cause included.

Learning: In Zoo-Code-Org/Zoo-Code, the TypeScript tools src/core/tools/ApplyPatchTool.ts and src/core/tools/ApplyDiffTool.ts intentionally differ when an internal stable read finds a version newer than the prior observation. ApplyPatchTool records the current version as partial; ApplyDiffTool retains the older observation. In src/core/tools/guardedWrite.ts, partial observations reject full-file replacement but can authorize targeted edits, while older observations reject writes through the stale-version check. Do not infer that refreshing a token as partial grants full-file replacement authority, or require both tools to use the same prior-observation policy solely because their stat-bracketed reads look similar.

PostCommitDurabilityError means the commit rename landed but the
destination's directory entry is not known to be durable: after a crash
the destination may not survive. Unlinking the source on that path could
leave NEITHER path available, so the source is kept and the durability
error propagates - the model learns the move did not complete while both
paths still hold the content. Supersedes the previous round's completion
of the observable half (and its PartialMoveError-on-unlink-failure):
crash availability outranks not leaving the content at both paths, since
the duplicate is visible to the model and recoverable while a lost file
is not.

…'s lock
The move unlinked the source with a bare fs.unlink after the destination
landed: a peer that rewrote or removed the source in between lost its
bytes or the move reported a success it could no longer verify. The
deletion now runs under the source path's canonical advisory lock - the
same key the guarded publish takes - and re-checks the source's version
token against the observation the move was built from. A source that
vanished, changed, or has no usable observation is kept and the move
raises PartialMoveError naming the committed destination and the remaining
source; success and moved-file tracking are not reported. The durability
path already keeps the source outright, which is the same protection in
its strongest form. The destination lock is released by the publish before
the deletion, so the two locks are never held together; the sorted-key
order for a future combination is documented at the call.

Walkthrough carry-over — evidence against current head 440dca448
The walkthrough comment (updated 2026-10-16T04:34:30Z) still carries two ERROR rows whose claims predate this head (pushed 2026-10-16T03:55Z; required CI green at this head). Both repeat claims already fixed or dispositioned here:
Persistence Integrity — fixed at this head. The row cites "raw fs.unlink(absolutePath) at lines 541-545" — those are the line numbers at the superseded head 993fc6f. At 440dca448 the source deletion is serialized under the source path's canonical advisory lock (withFileLock on the canonical lock key — the same key the guarded publish takes), and under that lock the source's recorded observation version is re-checked against a fresh bigint stat: a source that is missing, changed, vanished, or lacks a usable observation is kept and the move raises PartialMoveError naming the committed destination and remaining source before any unlink, with no success report and no moved-file tracking update. The two locks are never held together (the publish releases the destination lock before the deletion begins); the code comment states the shared sorted-key order for any change that combines them. The durability-error path keeps the source outright — PostCommitDurabilityError propagates before the deletion is reached, the strongest form of the protected deletion the Resolution asks for. Tests: the never-observed case asserts no unlink and no tracking; new tests pin the changed-source rejection, the vanished-source rejection, and the lock key; negative controls (faithful mutants of each check) each redden exactly one test. 26/26 green.
Security Boundaries — registered disposition (warn-only DACL). The row re-flags the warn-only DACL restore in this branch's carrier copy of safeWriteText.ts (the block at lines 845-887 it cites). The fail-closed change belongs to the publish-primitive owner (PR 1916, fixed there in 3624e4fea), which merges before this unit in the declared order; this unit carries the same carrier copy and reconciles at rebase (record 6106959976 on issue 1408; tracking 1991). Implementing it twice here would fork the fix.
No code change this round: the row texts repeat the fixed and dispositioned claims verbatim; the walkthrough's incremental review did not re-evaluate these rows against 440dca448.
Disposition update for the 06:44Z walkthrough rows (comment 5994168415, head 440dca448)
Security Boundaries (Windows DACL restore-before-commit) - the row verbatim-repeats the Windows DACL family already dispositioned in this PR's registration sections: safeWriteText.ts is carrier content on this unit (owner: the safeWriteText publish-primitive owner unit), and the DACL restore/commit ordering is that primitive's contract - the fix lands once at the owner and copies reconcile at rebase (precedent record 6106959976, issue #1408). Evidence grade: registered-family repeat on carrier content; no new concrete claim versus prior waves.
Persistence Integrity (guarded move can delete newer source state) - the row describes the pre-fix code: it cites "raw fs.unlink(absolutePath) at lines 541-545", but at head 440dca448 lines 538-555 are the contract comment and the deletion at lines 556-587 already implements the row's Resolution verbatim: source deletion is serialized under the source path's canonical advisory lock (withFileLock(await resolveLockKey(absolutePath))), the source's version token is re-checked under that lock against the observation the move was built from, and a source that vanished, changed, or has no usable observation is kept with PartialMoveError naming the committed destination and the remaining source - no success is reported and moved-file tracking is not reached (the throw precedes it). The two locks are never held together (the destination lock is released by the publish first), with the shared sorted-order requirement documented at the lock site; the durability-error path keeps the source outright (lines 516-521). The row's claim that "the move tests cover unlink failure but not source changes" is also stale: applyPatchTool.execute.spec.ts carries "keeps the source when it changed after the destination was published" (L707), "keeps the source when it vanished before the deletion" (L742), and "deletes the source under the advisory lock for the source path" (L772). Evidence grade: stale row citing pre-fix line numbers; fix and tests present at the current head.
No code change this round; heads unchanged (440dca448).

… key's referent
resolveLockKey returns the symlink referent for a symlinked source, and the cleanup used it for stat and unlink: a move of a symlinked file deleted the referent instead of the link. The lock is still taken on the resolved key (so it serializes with writers of the real target), while stat and unlink now use the source path. The lock-key test also asserts the exact key and that nothing but the source path is unlinked.
…he extended fs double
The production dead-code check reports resetChain as unused (only tests reset the chains); @internal is the supported mechanism. The execute spec's fs mock now declares the lstat/readlink/realpath doubles its symlinked-source case uses, so check-types passes.

Assert that the source lock is held during stat and unlink.
The move tests check withFileLock call arguments and unlink call arguments only after tool.execute returns. The withFileLock mock runs the operation directly and does not record which keys are held. If stat or unlink moved outside the lock callback in src/core/tools/ApplyPatchTool.ts, these assertions would still pass. The tests would then miss the loss of serialization with peer writers that their own comments describe.
Record the held keys in the mock. Assert that the key for the operated path is held when stat and unlink run. In the symlink test, that key is referentKey. In the plain move test, it is sourceKey. Test guidelines for this area require in-flight behavior to be asserted during execution, not only after the call completes.

around lines 836 - 837:
Update the move tests’ `withFileLock` mock to record which lock keys are held
while its callback runs, then assert during execution that the path’s key is
held when `stat` and `unlink` run: `referentKey` in the symlink test and
`sourceKey` in the plain move test. Keep the existing post-execution argument
assertions.

This PR routes the
apply_patchtool through the guard, so a patch hunk can no longer "write" a file the model never fully read: an unobserved overwrite and a stale version token are refused with the standard remediation. A move carries the source's completeness to its destination instead of claiming completeness for lines the model never read, and a partial-source move onto an already-observed destination is rejected before any state changes. It also fixes a resource leak: when the guard refuses a publish, saveDirectlynow rolls back the parent directories it created. It is the sixth of nine PRs for epic #1375 (chain tracked on #1991).

Description
What this PR changes:
apply_patchpublishes viaguardedWrite, so an unobserved overwrite and a stale version token are rejected with the read-first / re-read-then-retry remediation instead of clobbering the file.completeflag, so a slice, range, truncated, or indentation-block source never authorizes a full-file replacement at the new path.saveDirectlycaptures the listcreateDirectoriesForFilereturns and, if the guard rejects, removes those directories innermost-first withrmdir(which refuses a directory another writer populated, so the loop stops at the first failure) and rethrows the original write error.AncestorReplacedErrorif an ancestor was swapped underneath it.Why the diff looks bigger than the change: the chain is non-cumulative - this branch is based on
mainand carries its own copy of the earlier units' content, which the GitHub diff shows as context.Review focus:

Stryker directive removal (equivalence notes)
Per AGENTS.md the two Stryker suppression comments in the guarded save
path were removed instead of kept; their equivalence claims:
"update"->""): an empty kinddispatches exactly like
"update"inside guardedWrite - only"edit"and
"create"branch distinctly - and the default-parameter line isnot part of this unit's changed-line selector, so the mutant is
equivalent / out of scope.
GuardRejectedErroris only thrown by guardedWrite, which is reachedonly after the
!saveTaskcheck above has already thrown. Wheneverthe operand is evaluated
saveTaskis provably non-null, so droppingthe operand cannot change which branch is taken.
mutation-diff remains advisory for stacked units; maintainers decide any
narrow gate-config exclusion.

Re-review responses (error-fix re-review wave)
answered by completing the source unlink and reporting unlink failures as
PartialMoveError- since SUPERSEDED by the crash-availability reversalbelow; see the next section for the shipped semantics.
Crash-availability reversal (durability-error move)
A follow-up review (Major Data-Integrity) reversed the previous round's
guidance for the same branch, and the new semantics shipped: when the
destination's commit rename landed but its directory entry is not known to
be durable (
PostCommitDurabilityError), the source is KEPT and thedurability error propagates - unlinking it could leave NEITHER path
available after a crash.
The move now
deletes under the shared advisory lock (
withFileLockon the canonicallock key): the source must still be present in the observation registry
and its recorded version must equal a fresh bigint stat of the source,
otherwise the move raises
PartialMoveErrorbefore any unlink. A statfailure is likewise a
PartialMoveError- an unreadable source is not adeletable source.
Durability note: on the durability-error path the source is kept
outright -
PostCommitDurabilityErrorpropagates before the deletion isreached - the strongest form of the protected-deletion contract this
unit's issue asks for.
The PI row's raw-unlink line numbers belong to the
superseded head
993fc6f; at this head the deletion is lock-serializedand observation-bound exactly as the Resolution demands (missing,
changed, vanished, or unobserved sources are kept;
PartialMoveErrornames the committed destination and remaining source; the durability
path keeps the source outright; tests and negative controls above). The
SB row re-flags the carrier copy's warn-only DACL restore, whose
fail-closed fix lands at the publish-primitive owner (PR 1916,
3624e4fea) that merges before this unit; copies reconcile at rebase(record 6106959976 on issue 1408). Evidence comment posted on the pull
request; no code change this round.
