Tracking issue: 1391

Summary

S3 of the file-write safety series (plan: retired fork tracking item 33 (retired fork tracking item 33)), part of epic 1375. Introduces the atomic text publish primitive (A4): agent file writes now go temp → fsync → close → atomic rename, so a crash or power loss mid-write can never leave a torn file at the target path. The primitive generalizes the staging/backup/rollback logic currently inline in safeWriteJson (refactored to delegate to it), and DiffViewProvider.saveDirectly (the path all five write tools use) switches from raw fs.writeFile to it.

Changes

  • src/services/file-safety/safeWriteText.ts (new): safeWriteText(filePath, content, options?)
    • Writes content to a temp file in a private per-write staging subdir (same volume → atomic rename), fsyncs the fd before close, then atomically renames temp → target.
    • backup: true keeps the old-file semantics (target → backup before commit; backup deleted on success, restored on failure); default is plain atomic replace.
    • Symlinks: resolves the target via fs.realpath first (falling back to the given path when it does not exist), so a write through a symlink replaces the referent's content and never replaces the link itself.
    • Windows DACL: saves the target's DACL to a dump via icacls /save <target> /T before the backup rename, then restores it onto the target's directory after the commit rename. Any failure skips DACL handling entirely (the write is never blocked) and the dump file is always unlinked.
    • File descriptors are released in try/finally, so a failing write/fsync never leaks one.
    • Injection points (platform, execFileRunner, tempPath) keep both platform branches testable without a Windows runner; tempPath lets a caller pre-write (streaming) then fsync+commit.
  • src/utils/safeWriteJson.ts: the commit step now delegates to safeWriteText with the pre-written stream temp (tempPath) and backup: false (the JSON path already manages its own backup); rollback/cleanup logic unchanged.
  • src/integrations/editor/DiffViewProvider.ts: saveDirectly writes via safeWriteText instead of raw fs.writeFile — crash/power-loss safe for every agent write.

Tests

  • New safeWriteText.spec.ts: staging/fsync/close/rename ordering; torn-write failure leaves the target byte-identical with no temp behind; backup rollback restores the old file; target-absent with backup commits without a backup; win32 DACL save-before-rename / restore-after-commit ordering, the skip-entirely path when the target is absent, and dump cleanup on failure; a symlink-resolution test (runs on all platforms) proving the commit rename targets the realpath result (the referent) and never the link path, plus a realpath-failure fallback case; pre-written tempPath commit.
  • DiffViewProvider.spec.ts: save-path assertions moved from raw fs.writeFile to the mocked safeWriteText primitive.
  • safeWriteJson suite unchanged (behavior-preserving refactor).
  • ESLint clean; suppression counts unchanged; check-types clean.

Notes

  • Behavior-preserving for all successful writes: same files end up at the same paths. The safety gain is crash/power-loss atomicity (zero torn-window) and a private staging dir that keeps concurrent writes from colliding.
  • No version guard yet — that is S4 (A3), which consumes this publish step.

Review-gate re-trigger (2026-08-30): empty commit 7fd49bc (no code change) re-runs CI and CodeRabbit current-head review under the org new PR review gate; the code head remains a37dd24.

Split-unit issue reference

An approved upstream unit tracking issue exists and is linked at the top of this description: #1391 (Tracking S3: atomic text publish primitive (A4) - safeWriteText). This pull request is one unit of a declared split of a larger change; the split plan, the unit boundaries and the cross-unit review dispositions are recorded on the chain tracking issue #1991. The link is deliberately non-closing: #1391 is resolved by the maintainer when the unit merges.

Authorized-root binding (53df674)

The 10-11 Security Boundaries row implemented at this unit (the publish-primitive owner): safeWriteText takes an optional authorizedRoot; a requested path that resolves lexically inside the root must also publish inside it - a dangling or existing symlink whose referent escapes the root is refused before any mkdir or write, so an approval that covered the inside path can never be spent on an outside target (the row's approved.txt -> /tmp/outside/new/secret.txt scenario). Requested paths already outside the root stay governed by the tool layer's own approval flow; a root that cannot be inspected fails closed. DiffViewProvider.saveDirectly passes the task cwd as the root. Tests: 7 new (dangling/existing escapes refused before mkdir+rename, inside referents published, outside-requested and no-root cases unchanged, lexical-root fallback, EACCES propagation); four negative controls pin each element. 95/96 unit (1 pre-existing skip), 75/75 diff-view; the single integration failure is the pre-existing win32 icacls/restore host limitation, identical on the clean tree.

Review round final head (9601933)

Refreshed onto main 4f1dee18e (merge 0db5ccec9, no conflicts; lint-gate commit drops the SafeWriteTextOptions import main's side orphaned). CodeRabbit's three new threads on the authorized-root commit are fixed and resolved at 960193379: symlinked-parent-directory bypass (target canonicalized to its nearest existing ancestor), same-path staging recovery (fresh-name recovery orphaned a directory and re-ENOENTed), and per-suite mock defaults (filtered -t runs of the top-level suites crashed before). 98/99 unit (1 pre-existing skip); each fix has a negative control reddening exactly its test.

📝 Summary

Summary by CodeRabbit

  • Bug Fixes
    • Editor and JSON saves now publish complete file contents, reducing the risk of partial writes.
    • Saving through a symbolic link updates its target while preserving existing permissions; new files receive standard permissions.
    • Editor saves reject access failures on existing files while still allowing missing files to be created.
    • Editor saves are blocked when a link’s target escapes the authorized working directory.
    • Failed saves clean up temporary files and preserve existing content where possible.
    • On Windows, saves retry certain file-access errors. Permission-restoration failures may be reported after new content is published.
    • A save may report a durability error after publishing if the system cannot confirm the change is durable.
    • File locks recognize symbolic-link aliases, preventing concurrent writes through different paths to the same file.

The change adds safeWriteText for staged file publication and routes editor direct saves and JSON writes through it. It resolves symlink targets, checks authorized roots, handles permissions and platform-specific durability, and canonicalizes file-lock paths. Cache clearing also handles errors raised after a write has committed.

Full details: Title check

Explanation

The title names the primary change: an atomic text publish primitive (safeWriteText) and a refactor of safeWriteJson onto it. It matches the changeset, which adds src/services/file-safety/safeWriteText.ts, refactors src/utils/safeWriteJson.ts to delegate to it, and routes DiffViewProvider.saveDirectly through it. The title is 85 characters and 10 words, which is long but still specific. It also uses a conventional-commit prefix and an issue reference, which are acceptable.

Full details: Description check

Explanation

The description covers the core content: summary, change list, tests, and notes. It links the tracking issue (#1391) and explains the atomic publish design, the DACL handling, and the authorized-root binding. It does not follow the repository template. The template's Pre-Submission Checklist, Test Procedure, and Documentation Updates sections are missing. The Related GitHub Issue section uses a non-template format. The body also includes long status-log content: a re-trigger note, a split-unit note, and review-round notes with commit hashes. The writing also departs from the Simplified Technical English style the template requests, with long sentences and several dense clauses. The description is mostly complete on substance, so it passes, but the template sections should be added and the review-round history should be trimmed or moved to comments.

Full details: Regression Evidence

Explanation

The prior staging-directory regression is covered. safeWriteText.spec.ts:2001-2024 forces fsSync.mkdirSync to throw both EACCES and ENOSPC, verifies the exact error, and verifies that openSync and rename are not called. The changed publish, symlink, authorization, durability, DACL, cleanup, lock, JSON, editor, and path-resolution behaviors also have focused tests, including real-filesystem integration coverage. No durable visible UI change requires a Playwright component snapshot. Prior failure addressed: The current focused suite adds staging directory creation failure tests at safeWriteText.spec.ts:2001-2024 for EACCES and ENOSPC, with exact-error, no-open, and no-rename assertions.

Full details: Lifecycle Resource Cleanup

Explanation

No changed lifecycle path shows a listener, watcher, timer, task, or provider leak. safeWriteText closes file and directory descriptors in finally blocks (lines 562-577, 597-605, 701-706), awaits and completes rename retry timers (683-694), and cleans staging, temporary, and DACL artifacts on success and failure (750-802). safeWriteJson releases its advisory lock in finally (184-190). The changed saveDirectly path adds no persistent resource. The stream listener setup in _streamDataToFile is unchanged from the base revision.

Full details: Security Boundaries

Explanation

The new authorized-root check is path-based and has a TOCTOU gap. safeWriteText validates the target at lines 420-455, then creates dirPath with fs.mkdir(..., { recursive: true }) at lines 469-472 and later renames through that path. If a concurrent process replaces an approved workspace parent with a symlink to /outside after validation, staging and the final rename follow the new symlink and publish outside the authorized root. saveDirectly has the same gap between its pre-check and createDirectoriesForFile at lines 1163-1173. This bypasses the new workspace allowlist for an approved in-root path.

Resolution

Make authorization and publication race-resistant. Open or retain directory handles for the authorized root and target parent with symlink traversal disabled, create the staging directory through those handles, and perform the final rename relative to the verified parent handle. At minimum, reject or revalidate every parent component immediately before publication; do not rely on string-path realpath checks followed by path-based mkdir and rename.

Full details: Persistence Integrity

Explanation

The cache-manager case is fixed, but another changed safeWriteJson path still loses state on a post-commit error. safeWriteText renames the staged file before it reports PublishNotDurableError (safeWriteText.ts:678-712, 804-809). safeWriteJson rethrows that error (safeWriteJson.ts:148-183). TaskHistoryStore.writeTaskFile updates its written value during the merge, but upsertCore updates its in-memory cache only after writeTaskFile resolves (TaskHistoryStore.ts:865-881, 239-253). If the directory fsync fails after rename, disk contains the new history item while the cache retains the old item. A later task-history save or administrative repair can use that stale state and overwrite or omit persisted fields.

Resolution

Handle PublishNotDurableError and DaclRestoreError as committed writes in every safeWriteJson consumer with in-memory state. In TaskHistoryStore.writeTaskFile and its callers, retain the merged written value, update the cache before propagating the post-commit error, and prevent a retry from writing stale content. Apply the same policy to saveTaskMessages/saveApiMessages callers and any MCP or configuration state that retains the written object. Add failure-injection tests that fail the directory fsync after rename, verify the target contains the new data, verify in-memory state is updated, and verify a subsequent save cannot restore stale data.

Use a typed error-code guard.

Replace the assertion with an unknown type guard that verifies code is a string. This removes the undocumented cast.

As per coding guidelines, “If an unavoidable cast is required, document why in a nearby comment.”

Assert the operation order.

These assertions check call counts only. The test still passes if rename runs before fsyncSync or closeSync.

Record each mock operation in an array. Assert this exact sequence:

openSync → writeSync → fsyncSync → closeSync → rename

As per coding guidelines, use unit tests for pure logic and state transitions.

Remove the skipIf guard on the win32 DACL test.

The test passes platform: "win32" and uses the mocked execFile, so it does not need a Windows host. With it.skipIf(process.platform !== "win32") the test never runs on Linux or macOS CI. Remove or unskip the skipIf Windows DACL test.

safeWriteText accepts a platform override, so this test does not need a Windows runner. it.skipIf(process.platform !== "win32") makes it dead on every Linux and macOS lane. The tests at lines 285-311 already assert the same icacls save and restore calls with platform: "win32". Delete this case, or drop the skipIf guard so it runs everywhere.

…t boundary and staging

- authorizedRoot bypass (Major): a NEW file under a symlinked parent
directory escaped the root unchecked - the retained target of a
not-yet-existing path is lexical, so root/sublink/new.txt looked
inside while the bytes landed at /outside/new.txt through the
committed parent symlink. The inside-target test now canonicalizes
the target's nearest existing ancestor and re-appends the missing
tail, so the check sees where the bytes land, not where the name
sits. Regression test: dangling write under a parent link out of the
root is refused before mkdir. Negative control: reverting to the
lexical target reddens exactly that test.
- staging recovery leak (Minor): the ENOENT reopen recovery called
_stagingDir() again, which generates a FRESH random name - the
recreated directory did not contain tempPath, so the reopen failed
again on a real filesystem and the new directory was orphaned. The
recovery now recreates the same stagingDir path and verifies it is a
directory before reopening. The recovery test now pins that the
recreated path equals the temp file's parent; reverting the fix
  reddens exactly that test.
- suite isolation (Minor): the two top-level suites without a
beforeEach inherited mock defaults from the main suite's beforeEach,
so a filtered run (-t) crashed with TypeError. The defaults moved to
a shared _restoreFsDefaults() called by every suite; both filtered
runs now pass standalone and fail without the beforeEach.

saveDirectly ran createDirectoriesForFile before safeWriteText's
authorizedRoot check, and createDirectoriesForFile follows symlinks: an
approved inside path whose intermediate link points outside the workspace
mutated the outside filesystem before the publish was rejected. Export the
canonical ancestor check from safeWriteText and run it first, so the
allowlist decision has no filesystem side effects (Security Boundaries

…d lock the referent

Review follow-ups: (1) a host whose icacls /restore exits 1300 now publishes successfully with one process warning instead of throwing DaclRestoreError after the rename on every overwrite; non-privilege restore failures still throw; (2) DiffViewProvider.saveDirectly treats PublishNotDurableError and DaclRestoreError as a completed save with a warning so the tool layer does not retry content already on disk; (3) file locks canonicalise through resolveRealPath (nearest existing ancestor, case preserved) so TaskHistoryStore and safeWriteJson serialize on the same lock through a symlinked ancestor; (4) safeWriteText reuses WorkspacePathResolver.resolveRealPath via a caseFold option instead of a private copy.
Review follow-up: the durability fsyncs (staged file and parent directory) now use the async fs.fsync form instead of fsyncSync, so the ~7 ms syscall no longer blocks the extension host event loop; this is the same syscall FileHandle.sync() uses internally. fsync failures still reject before the rename, and ordering assertions still prove file-fsync-before-close and directory-fsync-after-rename. Tests 98 passed.
Review follow-up: no production caller passes backup: true, and the option added a copy/retain/cleanup path with its own ~20 tests. Remove it until a caller needs the recovery copy: the primitive keeps the single atomic rename, staging fsync, DACL capture/restore and the dump cleanup. Tests and comments are pruned accordingly (the DACL retry, lost-DACL and ordering coverage stays). Related suites 200 passed, 8 skipped.
The durability test stubbed fsyncSync to fail the parent-directory barrier; the primitive now uses the async fsync form, so the test wraps fs.fsync and fails the second call through its callback. Both safeWriteJson and safeWriteText suites pass (110 passed, 8 skipped).
Windows CI returns an 8.3 short os.tmpdir() path, but resolveRealPath returns the long form, so the lock-path assertions now compare against the realpath of the existing ancestor instead of path.resolve. 3 tests passed locally.

All six review threads resolved; head is green (6/6)

215006153 addresses every open thread:

  1. Host-limitation DACL restore now warns once and publishes instead of throwing after the rename; non-privilege restore failures still throw.
  2. DiffViewProvider.saveDirectly treats post-commit PublishNotDurableError/DaclRestoreError as a completed save with a warning.
  3. File locks canonicalise via resolveRealPath so TaskHistoryStore and safeWriteJson share one lock through a symlinked ancestor (new fileLock.spec.ts covers the alias case).
  4. The private nearest-ancestor walk reuses WorkspacePathResolver.resolveRealPath with a new caseFold option.
  5. The durability fsyncs run on the libuv thread pool instead of blocking the event loop.
  6. The unused backup option and its copy/retain/cleanup path are removed.

Related suites: 110 passed (plus the broader file-safety set 200 passed, 8 skipped). Windows/Ubuntu platform checks are green again after the 8.3-path fix in the new lock test.

Transient restore failures must retain retries and DACL handling for later writes; they must not set the process-wide memo. Once the missing-privilege memo is set, skip DACL capture as well as restoration and emit the privilege warning only once.

…s for cache state

Review follow-up: add isPublishCommittedError(error) for PublishNotDurableError/DaclRestoreError and use it where a consumer keeps state derived from the write. CacheManager.clearCacheFile now clears the in-memory hashes when the empty cache committed but its durability/ACL verification failed (the file on disk is the new one); DiffViewProvider reuses the shared guard. safeWriteText keeps the exact-commit failure contract. Adds slice tests: guard classification, staging mkdirSync EACCES/ENOSPC without open/rename, and the committed-clear cache path. Related suites 205 passed, 8 skipped.

Both rows addressed in 69d270779

Persistence Integrity (error) — post-commit error could lose cache state. Added the shared guard isPublishCommittedError(error) (true for PublishNotDurableError/DaclRestoreError) in the publish primitive and re-exported it alongside safeWriteJson:

  • CacheManager.clearCacheFile now clears fileHashes when the empty cache was committed but its durability/ACL verification failed — the file on disk is the new one, so keeping old in-memory hashes would disagree with it. A pre-commit failure still keeps the old state and the existing error telemetry.
  • DiffViewProvider.saveDirectly reuses the same guard instead of its own instanceof pair.
  • Tests: guard classification (both post-commit classes accepted, pre-commit/plain errors rejected) and a clearCacheFile test that mocks PublishNotDurableError and asserts the hashes are cleared (cache-manager 12 passed).

Regression Evidence (warning) — staging-dir mkdir failure untested. Added EACCES/ENOSPC cases: safeWriteText rejects with the exact mkdirSync error and never calls openSync or rename.

Related suites (safeWriteText, integration, safeWriteJson, cache-manager, DiffViewProvider): 205 passed, 8 skipped. Required checks re-run on the new head.

src/services/file-safety/safeWriteText.ts:778-779
🗄️ Data Integrity & Integration | 🟡 Minor | ⚡ Quick win

Preserve caller-supplied temp files on pre-commit failure.

When options.tempPath is set, the DACL-capture path treats the file as caller-owned at Line 635. The outer rollback still unlinks tempPath for every pre-commit error. A failed fsync, rename, or mode operation can therefore delete the caller's staged file. Skip this unlink for caller-supplied paths, or apply one ownership rule consistently. Add a regression test for a caller-supplied temp path and a forced pre-commit failure.

Update the outer rollback cleanup in the safe-write flow to preserve
caller-owned temporary files when options.tempPath is supplied, while retaining
cleanup for internally created paths. Apply the same ownership rule used by the
DACL-capture path, and add a regression test that forces a pre-commit failure

📐 Maintainability & Code Quality | 🔵 Trivial | ⚡ Quick win

Use a typed no-link filesystem mock.

The default fs.lstat mock returns null as unknown as fsSync.Stats. This bypasses strict typing and does not match the runtime return shape. Mock an ENOENT rejection or provide a typed non-symlink Stats object instead.
As per path instructions, TypeScript changes must use strict typing and introduce no unjustified double assertions.

Update the `fs.lstat` mock to use a runtime-valid, strictly typed result: have
it reject with an `ENOENT` error or return a typed non-symlink `Stats` object.
Remove the `null as unknown as fsSync.Stats` double assertion.

Learning: For Zoo-Code-Org/Zoo-Code PR #1395, src/utils/safeWriteJson.ts intentionally supports leaf-symlink targets by resolving and publishing at the referent. Locking, merge reads, staging, and publication use the canonical path to preserve the alias and coordinate alias/referent writers. Do not require blanket rejection of leaf symlinks. The user assigns storage-root confinement to confineTo in file-safety units U4/U6, tracked separately in easonLiangWorldedtech/Zoo-Code#41. The cited _refuseSymlinkedAncestors guard at commit 17c736ecb is an opt-in refuseSymlinkTarget policy and is not present at PR #1395 head 6df83d845.

Learning: In src/integrations/editor/DiffViewProvider.ts, DiffViewProvider.saveDirectly intentionally requires a writable parent directory because it uses safeWriteText for atomic rename-based publication. Do not request an in-place fs.writeFile fallback for a writable target in a non-writable directory; that fallback would discard the intended atomicity. The caller separately checks fs.access with fsConstants.W_OK to reject existing non-writable targets and permits ENOENT for new targets.

Comment thread src/services/code-index/__tests__/cache-manager.spec.ts Outdated
Review follow-ups: the alias/referent lock test now asserts the ELOCKED failure kind (lock contention, not an unrelated lockfile error) and releases both the holder and, should it ever acquire, the contending lock from a finally block so a failing assertion cannot strand the lock. The committed-clear cache test keeps the PublishNotDurableError instance and asserts the warning receives the exact message prefix plus that instance. fileLock + cache-manager specs 15 passed.
