ci: surface the changed-code mutation report as a job summary and a named artifact - #2149
Follow-up to #2050. First step of the "make the mutation report accessible to agents" direction raised in the review of #2051.
Why
#2051 proposed forbidding Stryker suppression comments in source. It was green (6/6 required checks, CodeRabbit approved, 0 open threads), but the blocking review asked for a different approach. Key sentence of that review (edelauna, CHANGES_REQUESTED, 2026-10-18T03:49:28Z on #2051):
I'm not sure I want to take this approach just yet - there are some valid reasons for disabling Stryker. If anything the approach would be to make the report generated by Stryker more accessible for agents, because as it stands agents might not be aware of the findings, unless they navigate into the annotations, and then agents might not even know they can download the report as an artifact on the ci stryker job - I'm leaning towards closing this PR and approaching this differently.
#2051 was then closed by the author on 2026-10-22T09:57:41Z (head 44ca22216) and nothing from it landed on main. This PR takes the suggested direction instead: surface the findings so a reader (agent or human) cannot miss them.
This PR does not change the Stryker suppression policy. It adds no prohibition and no enforcement: Stryker disable comments remain allowed exactly as AGENTS.md describes them today, and the mutation-diff job stays advisory.
What changes
1. .github/workflows/mutation-testing.yml — adds a Summarize per-file mutation results step directly after the mutation run. It parses every reports/mutation/<package>/mutation.json the job already produces and appends a per-file table to $GITHUB_STEP_SUMMARY:
Rows are sorted by Survived + NoCoverage, worst file first, so the files that need attention are at the top of the job summary — no artifact download, no annotation hunting. Score is Killed / Mutants, counting Timeout mutants as valid but not killed (stated in the summary itself). Paths are resolved against the report's projectRoot, so rows read src/utils/pathUtils.ts. Missing or unreadable reports degrade to a one-line note, and the summary write itself is guarded (a failure there emits a ::warning instead of failing the step), so the step cannot turn the advisory job red. It deliberately adds no second continue-on-error: scripts/stryker-diff.test.mjs asserts the workflow has exactly one, and mutation-diff runs that suite.
The step adds no dependency and writes no new files: it reads the JSON reports the gate already generates. The uploaded artifact was already named changed-code-mutation-report and stays exactly that — one upload, not a second identical one.
2. AGENTS.md — a short operational section, "Reading the changed-code mutation report": the job summary lists per-file Survived/NoCoverage worst-first plus every surviving/uncovered mutant; the same data is in the changed-code-mutation-report artifact (reports/mutation/<package>/mutation.json and mutation.html, 7 days); close a listed mutant with a focused test that fails under the mutation. The existing Stryker disable reason rule is referenced, not restated or changed.
No .changeset file and no CHANGELOG edit (repo rule: maintainers manage both).

The step's script was extracted from the committed YAML exactly as bash will see it and run against a real Stryker report from the #2051 reproduction (mutation.json with 4 mutated files, 44 mutants, 2 timeouts), with GITHUB_STEP_SUMMARY set:
### Per-file mutation results
Every file below comes from the `changed-code-mutation-report` artifact (`reports/mutation/<package>/mutation.json`). Rows are sorted by Survived + NoCoverage, worst first; Score is Killed / Mutants, counting Timeout mutants as valid but not killed.
| File | Mutants | Killed | Survived | NoCoverage | Score |
| --- | ---: | ---: | ---: | ---: | ---: |
| `src/utils/pathUtils.ts` | 33 | 22 | 9 | 0 | 66.7% |
| `src/api/providers/utils/abort-signal.ts` | 3 | 1 | 2 | 0 | 33.3% |
| `src/core/assistant-message/NativeToolCallParser.ts` | 4 | 2 | 2 | 0 | 50.0% |
| `src/core/webview/webviewMessageHandler.ts` | 4 | 4 | 0 | 0 | 100.0% |
Close a surviving or uncovered mutant with a focused test that fails under the mutation; the artifact does not need to be downloaded to read this table.
Degradation paths checked: no reports/mutation directory → one line, No Stryker JSON report was produced for this run; corrupt JSON → the table is omitted and the package is named in Unreadable report(s): ...; GITHUB_STEP_SUMMARY unset → the summary goes to stdout; the step exits 0 in every case.
Formatting:

OK .github/workflows/mutation-testing.yml
OK AGENTS.md
$ git diff --check
(no output, exit 0)
Run history note: the first mutation-diff run on this PR was red — the new step initially carried continue-on-error: true, and scripts/stryker-diff.test.mjs asserts the workflow has exactly one occurrence (the artifact upload). The second commit removes it and guards the summary write instead; node --test scripts/stryker-diff.test.mjs is 44/44 green locally after the change.
The changed-code mutation gate already writes a package-level summary and
uploads reports/mutation/ as the changed-code-mutation-report artifact, but a
reader had to open the artifact (or the annotations) to find which files still
have surviving or uncovered mutants.
Add a step after the mutation run that parses each package's mutation.json and
appends a per-file table (File | Mutants | Killed | Survived | NoCoverage |
Score) to GITHUB_STEP_SUMMARY, sorted by Survived + NoCoverage, worst first.
Document where to read it in AGENTS.md. No policy change: the job stays
advisory and no suppression rule is added or altered.

📝 Summary
Summary by CodeRabbit
New Features
Pull request summaries now show per-file mutation results, mutation scores, and counts of surviving or uncovered mutants. Results are ordered by those counts, with file paths breaking ties.
When no report is available, the summary says so; unreadable reports are noted. Detailed JSON and HTML reports are available as artifacts for seven days.
Documentation
Added guidance for interpreting mutation results and addressing reported mutants.
The pull-request workflow now summarizes per-package Stryker JSON reports by file. The script validates report paths and data, counts recognized mutant statuses, and writes a Markdown summary. Tests cover report processing and output writing. AGENTS.md describes where to find and how to use the results.
Changes: Mutation reporting
Layer / File(s)
Summary
Report validation and summary generation scripts/mutation-summary.mjs, scripts/mutation-summary.test.mjs
The script validates report data and paths, counts recognized statuses, and sorts file rows by surviving and uncovered mutants. Timeout counts toward the total and score numerator, but not the Killed column. Tests cover report validation, ordering, unsafe paths, and Markdown escaping.
Summary output and cleanup scripts/mutation-summary.mjs, scripts/mutation-summary.test.mjs
The script appends summaries atomically to a configured file or writes to stdout. It warns on write failures and removes temporary files. Tests cover output behavior and temporary-file cleanup.
Workflow integration and usage guidance .github/workflows/mutation-testing.yml, package.json, AGENTS.md
The pull-request workflow runs the summary script. The CI test command includes the new tests. AGENTS.md describes the job summary, seven-day artifact, and mutant follow-up guidance.

Explanation
The description explains the issue context, implementation, scope, verification steps, and checklist status. It includes a linked issue reference and detailed test evidence. The Testing checklist item is omitted, but the Verification section provides the required testing information.
Full details: Security Boundaries
Explanation
No changed path meets the stated security failure conditions. The new workflow invokes a fixed local Node script without shell interpolation. scripts/mutation-summary.mjs accepts report JSON only for fixed status counts and validates report paths against cwd, rejects traversal and unsafe characters, and escapes rendered Markdown. It emits fixed diagnostics for malformed reports. It does not read or expose secret values, execute report content, or change approval and allowlist controls.
Full details: Persistence Integrity
Explanation
No changed persistence path violates the check. The new job-summary path reads the existing summary, writes the combined content to a same-directory temporary file, and replaces the target with fs.renameSync; failed writes remove the temporary file and writeMutationSummary emits a warning without overwriting the existing summary. The workflow invokes this synchronous writer in a separate step, and the other changed files do not add persisted defaults or asynchronous writes.
Full details: Regression Evidence
Explanation
The prior regression evidence remains unresolved. The Timeout fixture asserts only internal row counts and a loose 50.0% match; it does not assert the rendered row's Mutants, Killed, Survived, NoCoverage, and Score cells. The workflow invokes node scripts/mutation-summary.mjs, but the tests import functions and do not execute the direct-entry guard with the default GITHUB_STEP_SUMMARY path. These are changed behaviors without focused coverage.
Resolution
Add a rendering assertion for the complete Timeout fixture row. Add a child-process test that runs node scripts/mutation-summary.mjs with temporary report and GITHUB_STEP_SUMMARY paths, then verifies output and exit success. Add workflow assertions for the summary command and pull-request always() condition if workflow wiring is intended to be regression-protected.
Full details: Lifecycle Resource Cleanup
Explanation
The new atomic summary path can leak a temporary file on workflow cancellation. appendSummaryAtomically creates the sibling at scripts/mutation-summary.mjs:197 and writes it at line 200, but its finally only runs while JavaScript continues; a SIGTERM during the write or before rename leaves the file. The cleanup sweep at lines 212-225 runs only after a successful rename. The workflow enables cancel-in-progress: true at .github/workflows/mutation-testing.yml:12-14, so a newer pull-request run can cancel this step. The added tests cover a successful sweep and cleanup after a rename error, but not cancellation or stale-file cleanup when the current append fails.
Resolution
Run stale-sibling cleanup before the append attempt. Wrap the complete append path in an outer try/finally, track whether the current temporary file was renamed, and unlink it whenever rename does not succeed. Add a test that creates a stale sibling, forces the current write or rename path to fail, and verifies that both the stale sibling and current temporary file are removed.

…inue-on-error
scripts/stryker-diff.test.mjs asserts the mutation workflow contains exactly one
continue-on-error (the artifact upload), so the new summary step cannot use it.
Guard the summary write and the report scan instead: a failure now emits a
::warning and the step still exits 0, keeping the advisory job non-blocking.
node --test scripts/stryker-diff.test.mjs: 44/44 pass.
When a package mutation run fails, scripts/stryker-diff.mjs can leave its package directory without mutation.json. The summary scan then executes continue, so the package is omitted. If other reports exist, the summary still renders a normal table with no indication that it is incomplete. Record the missing package as a warning while preserving the advisory, non-blocking behavior.

…he artifact
The job summary lists per-file Survived/NoCoverage counts; the per-mutant line/mutator/replacement detail lives in the changed-code-mutation-report artifact. The previous wording claimed the summary carried the detail, which the step does not emit.
Review findings on the inline summary: report.files keys were trusted and inserted into a Markdown table, and the logic had no committed coverage. The summary now lives in scripts/mutation-summary.mjs, which validates the report shape, normalises and validates every path (repo-relative, safe characters, no traversal, no NUL), escapes Markdown/backslash/CR/LF before rendering and skips unsafe entries with a note. scripts/mutation-summary.test.mjs covers status counting (including Timeout), sorting, path normalisation, empty/missing/malformed reports and the unsafe-path rejects; test:mutation-ci runs it in CI.
Review rows: no raw parser/filesystem error text in the summary (fixed bounded diagnostics, report names rendered only when slug-shaped); appendSummaryAtomically wraps temp creation/write/rename in one try/finally and sweeps stale temp siblings. Tests: malformed report asserts fixed diagnostic; stale sibling removal and rename-failure cleanup; 14 tests pass.
Both rows from the latest review are fixed in the new head
Security Boundaries: the summary no longer renders raw parser or filesystem error text. A malformed report records the fixed diagnostic <package>: invalid JSON, a directory read failure records reports/mutation: directory could not be read, and report directory names are only rendered when they match a simple slug (otherwise report).
Lifecycle Resource Cleanup: appendSummaryAtomically now wraps temp creation, write and rename in one try/finally: the temp file is unlinked whenever the rename did not happen (e.g. ENOSPC/failed rename), and a later run sweeps stale .summary.md.*.tmp siblings left by a cancelled run.
New tests: the malformed-report test asserts the fixed diagnostic (no parser text); a stale-sibling test and a rename-failure test assert no .tmp file survives. node --test scripts/mutation-summary.test.mjs: 14 tests pass.
The two rows describe the previous head; both are fixed in 1fa318a79
Security Boundaries: the head no longer renders any raw parser or filesystem error text. Malformed reports record the constant <package>: invalid JSON, a directory read failure records reports/mutation: directory could not be read, and report directory names are rendered only when they match a simple slug (otherwise report). See renderReportName and the fixed diagnostics in scripts/mutation-summary.mjs.
Lifecycle Resource Cleanup: appendSummaryAtomically wraps temp creation, write and rename in a single try/finally; the temp file is unlinked whenever the rename did not succeed, and a later run sweeps stale .summary.md.*.tmp siblings.
Coverage: node --test scripts/mutation-summary.test.mjs -> 14 tests, including a malformed-report test that asserts the fixed diagnostic (no parser text), a stale-sibling removal test, and a rename-failure test asserting no .tmp survives. Head is 6/6 green; please re-run the pre-merge checks against 1fa318a79.

…test
Review follow-up: the rename-failure test now uses an existing regular file as the target (so the initial summary read succeeds) and stubs fs.renameSync to fail once after the temp file is written. It asserts rename ran exactly once, the warning carries the EIO message, the temp file is removed, and the existing summary is untouched.
fix(skills): report YAML frontmatter parse errors and serialize SKILL.md safely #1321 — realpath failure in updateSkillModes: the resolution error now propagates instead of falling back to the discovered path; a resolution failure cannot write through a symlink (or replace it with a regular file). Added a test where discovery succeeds but the publish-time resolution fails: exact error propagates, no write/rename. SkillsManager.spec.ts 117 passed.
feat(file-safety): atomic text publish primitive + safeWriteJson refactor (A4, #1375) #1395 — lock contention + committed-cache warning: the alias/referent lock test asserts the ELOCKED failure kind and releases both the holder and, if it ever acquired, the contending lock from a finally block. The committed-clear cache test keeps the PublishNotDurableError instance and asserts the exact warning text + instance. fileLock + cache-manager 15 passed.
ci: surface the changed-code mutation report as a job summary and a named artifact #2149 — summary rename-failure test: uses an existing regular file (so the initial read succeeds and the write reaches rename), stubs fs.renameSync to fail once, asserts rename ran once, the EIO warning, temp-file removal, and that the existing summary is untouched. node --test scripts/mutation-summary.test.mjs passes.
feat(task): observation registry with read completeness (U3, #1375) #1912 — no-workspace fail-closed + exact staging dir: confineForMcpWrite throws when neither provider cwd nor getWorkspacePath() supplies a root, so a project write cannot proceed unbounded (regression test asserts safeWriteJson is never called); the backup-chmod test asserts fs.rmdir on the exact staging directory recorded by fsSync.mkdirSync. McpHub + safeWriteText 179 passed.
refactor(providers): map xAI tool_choice to the Responses API shape with a typed request body #2036 — abort listener leak on construction failure: the abort bridge now registers after the whole request body (including mapToolChoice) is built, so a malformed allowed_tools entry cannot leave a listener on metadata.abortSignal; the malformed-entry test asserts addEventListener was never called. xai.spec.ts 49 passed.

Key sentence of that review (edelauna,
CHANGES_REQUESTED, 2026-10-18T03:49:28Z on #2051):#2051 was then closed by the author on 2026-10-22T09:57:41Z (head
44ca22216) and nothing from it landed onmain. This PR takes the suggested direction instead: surface the findings so a reader (agent or human) cannot miss them. This PR does not change the Stryker suppression policy. It adds no prohibition and no enforcement:
Stryker disablecomments remain allowed exactly asAGENTS.mddescribes them today, and themutation-diffjob stays advisory. What changes
1.
.github/workflows/mutation-testing.yml— adds aSummarize per-file mutation resultsstep directly after the mutation run. It parses everyreports/mutation/<package>/mutation.jsonthe job already produces and appends a per-file table to$GITHUB_STEP_SUMMARY:Rows are sorted by
Survived + NoCoverage, worst file first, so the files that need attention are at the top of the job summary — no artifact download, no annotation hunting.ScoreisKilled / Mutants, countingTimeoutmutants as valid but not killed (stated in the summary itself). Paths are resolved against the report'sprojectRoot, so rows readsrc/utils/pathUtils.ts. Missing or unreadable reports degrade to a one-line note, and the summary write itself is guarded (a failure there emits a::warninginstead of failing the step), so the step cannot turn the advisory job red. It deliberately adds no secondcontinue-on-error:scripts/stryker-diff.test.mjsasserts the workflow has exactly one, andmutation-diffruns that suite. The step adds no dependency and writes no new files: it reads the JSON reports the gate already generates. The uploaded artifact was already named
changed-code-mutation-reportand stays exactly that — one upload, not a second identical one.2.
AGENTS.md— a short operational section, "Reading the changed-code mutation report": the job summary lists per-fileSurvived/NoCoverageworst-first plus every surviving/uncovered mutant; the same data is in thechanged-code-mutation-reportartifact (reports/mutation/<package>/mutation.jsonandmutation.html, 7 days); close a listed mutant with a focused test that fails under the mutation. The existingStryker disablereason rule is referenced, not restated or changed. No
.changesetfile and noCHANGELOGedit (repo rule: maintainers manage both).Diff: +92 / −0 across 2 files (
git diff --stat upstream/main...HEAD).Verification
Real YAML parse, two independent parsers:
The step's script was extracted from the committed YAML exactly as bash will see it and run against a real Stryker report from the #2051 reproduction (
mutation.jsonwith 4 mutated files, 44 mutants, 2 timeouts), withGITHUB_STEP_SUMMARYset:Degradation paths checked: no
reports/mutationdirectory → one line,No Stryker JSON report was produced for this run; corrupt JSON → the table is omitted and the package is named inUnreadable report(s): ...;GITHUB_STEP_SUMMARYunset → the summary goes to stdout; the step exits 0 in every case.Formatting:
Run history note: the first
mutation-diffrun on this PR was red — the new step initially carriedcontinue-on-error: true, andscripts/stryker-diff.test.mjsasserts the workflow has exactly one occurrence (the artifact upload). The second commit removes it and guards the summary write instead;node --test scripts/stryker-diff.test.mjsis 44/44 green locally after the change.Scope
